Each completed operation shall be signed by the Officer of Officers in charge of the operations concerned and each completed page shall be signed by master of the ship. PSCO observed 22 unsigned pages in ships Oil Record Book dating from 22AUG16 to 17OCT16. Expanded exam related to this deficiency and did not find any signs of malpractice. MARPOL (2011) Annex I/ Reg. 17.4.

Oil fuel pipes and their valves and fittings shall be of steel or other approved material. According to the vessel’s piping diagram, the gate valve for the bunker line should be made of a certified stainless steel. The gate valves on board the vessel are made of unapproved cast iron. PSCO found the oily water separator to be inoperable; during an attempt to test the system, the 15 ppm alarm arrangement defaulted to an error code and the oil content meter was incapable of displaying the oil content. PSCO also noted multiple leaks from the upper gasket of the oily water separator. 73/78 MARPOL 2011 Annex I/Reg 14.6. Code 30ac.
