All ships are required to be provided with a ship security alert system (SSAS) as described in paragraphs 2.12.4 and 2.12.15. Its intent is to send covert signal or message from a ship that will not be obvious to anyone on the ship who is unaware of the alert mechanism. Vessel's management self reported a failure of the DSAS during a monthly test. Equipment shall be repaired to original operations. Additionally ships management shall identify and alternative means of contacting shore-side support in the event of an emergency, and implement this procedure until repairs can be made. Without disclosing this procedure, notify our office once this has been put in place.

Surfaces with temperatures above 220°C which may be impinged as a result of a fuel system failure shall be properly insulated. Three (03) steam valves in fuel oil purifier room had deteriorated lagging. Replace insulation. 74 SOLAS (20) II-2/2.2.6.1

Oil fuel pipes, which, if damaged, would allow oil to escape from a storage, settling or daily service tank having a capacity of 500l and above situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. #2 SSDG and #1 SSDG remote fuel oil shut off valves seized. Crew conducted maintenance and rested with satisfactory results. 74 SOLAS (20) II-2/4.2.3.4

Plans and booklets shall be kept up to date; any alterations thereto shall be recorded as soon as practicable. Fire control plans and graphical symbols located at specific fire protection installations do not correspond and/or are missing (Manual call points marked as general alarms, missing fire damper symbols on cargo vents, machinery space emergency lighting not properly marked, etc.). Vessel is required to validate accuracy of fire control plan and update placards on fire protection installations to the satisfaction of class. 74 SOLAS (20) II-2/15.2.4.1 IMO Res A.952(23)

Portable fire extinguishers servicing 9.1 Extinguishers should be subject to periodical inspections in accordance with the manufacturer's instructions and serviced at intervals not exceeding one year. 9.1.2 All extinguishers together with propellant cartridges should be hydraulically tested in accordance with the recognized standard or the manufacturer’s instruction at intervals not exceeding ten years. Service extinguishers IAW one year/ten year requirement. 74 SOLAS 20 II-2/10.3 (A 23/Res.951)

Periodic Safety Test Procedures (aka Automation Test Procedures) must be developed, tested, and approved by USCG Marine Safety Center (MSC) or authorized classification society (ACS) under 46 CFR 8.420. Vessel was missing class or USCG approved PUMS to meet or reach equivalency of 46 CFR 62.50-30 for PUMS endorsement.Vessel is authorized PUMS in interim status, while procedure is completed and submitted to MSC or ACS. If ACS is utilized, ACS must certify to the OCMI the periodical safety tests have been approved on behalf of the U.S. Coast Guard and verified and satisfactorily demonstrated. 705: Due by next drydock (not including UWILD). 74 SOLAS 20 II-1 Part E 46 CFR Part 62

Engines installed on vessels that were constructed on or after January 1, 2000, with power output of more than 130 kW must be covered by a valid EIAPP certificate, certifying the engine meets the applicable emission standards of Annex MARPOL Annex VI. Provide documentation of completed EPA submission for EIAPP certificates to meet MARPOL VI/13. Provide EIAPP Certificates for all applicable marine diesel engines as proof of emissions compliance with the IAPP. 705: Due in 3 months to align with class condition of authority. 40 CFR 1043.30 MARPOL VI/13.8

Each bulk storage tank containing refrigerated liquefied CO2 for use aboard a vessel as a fire-extinguishing agent shall be subjected to a hydrostatic test of 1 1/2 times the maximum allowable working pressure in the tenth year of the installation and at ten-year intervals thereafter. After the test, the tank should be drained and an internal examination made. Parts of the jacket and lagging on the underside of the tank designated by the marine inspector must be removed at the time of the test so the marine inspector may determine the external condition of the tank. 705: Company shall make arrangements to complete at the next regularly scheduled Drydock. 46 CFR 61.10-5(g) NVIC NO. 01-13, CH-1 (1.14)
