Every steam pipe and every fitting connected thereto through which steam may pass shall be so designed, constructed and installed as to withstand the maximum working stresses to which it may be subject. 74 SOLAS (14) II-I/33.1 PSCO observed significant steam leak originating from heavy fuel oil purifier heater steam line valve located adjacent to purifier flat. Crew rectified on the spot.

Each incinerator shall be approved by the administration taking into account the standard specification for shipboard incinerators developed by the organization. Incinerator walls are to be protected with insulation fire bricks/refractory and a cooling system. MARPOL VI/16.6.1 MEPC.244(66) Incinerator had large pieces of refractory missing.
Action required: 40 - Rectify deficiencies prior to next US port after sailing foreign
Resolved 18 September 2019
Resolution: Master provided before and after pictures of incinerator refractory, a signed statement that repairs were completed, and a statement from class all attesting to the repairs.
