Whenever an accident occurs to a ship or a defect is discovered, either of which affects the safety of the ship or the efficiency or completeness of its life-saving appliances or other equipment, the master of the ship shall report at the earliest opportunity to the Administration, the nominated surveyor or recognized organization responsible for issuing the relevant certificate, who shall cause investigations to be initiated to determine whether a survey, as required by regs 7,8,9, or 10, is necessary. If the ship is in a port of another Contracting Government, the master or owner shall also report immediately to the appropriate authorities of the port State and the nominated surveyor or recognized organization shall ascertain that such a report has been made. USCG was notified by Pilots that while giving the command for astern propulsion the ship's engine did not respond by no RPM increase after two attempts were made. Vessel was being assisted by tugs and was able to successfully moor. Vessel required to submit a class report prior to movement.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Severe wastage was found throughout the cargo area that caused the PSCO to question the integrity of the cargo transfer and protection systems. PSCO noted severe deterioration of flanges, valves, supports, and lines of the cargo lines, vapor lines, electrical runs, P/V valves, and firefighting systems.

In tankers, electrical equipment shall not be installed in hazardous locations unless it conforms with the standards not inferior to those acceptable to the organization. 17 explosion proof lights were found with cracked lenses in the cargo pump room.

At a gas concentration above 30% LFL inside the steel enclosure the entire gas analyzing unit is to be automatically shut down. The gas detection system did not shut down when 30% LEL gas was applied to the sample point.

The gas detection equipment shall be so designed that it may be readily tested and calibrated. The crew applied 50% LEL span gas to the detectors but never received a detected value above 16%. The only cal gas bottle is expired.
