The purpose of regulation 4.1.1 of II-2 is to prevent the ignition of combustible materials or flammable liquids. For this purpose, the following functional requirements shall be met: means shall be provided to control leaks of flammable liquids. PSCO witnessed Auxiliary Engine # 2&3 with major F/O leaks in various locations. The Main Engine was leaking F/O on the # 2&3 cylinders as well.

The jacketed piping system shall include a means for collection of leakages and arrangements shall be provided with an alarm in case of a fuel line failure. PSCO observed major fuel leaks on #2&3 SSDG and the leak detection system drain valve was open allowing F/O to flow and bypass the leak detection system.

The Company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations with any additional requirements which may be established by the Company. Due to objective evidence in deficiencies 01 and 02 the vessel is not in substantial compliance with the relevant conventions, Calling into question the adequacy and implementation of the vessel's SMS under the ISM Come. A Safety Management Audit must be carried out in conjunction the Administration of the Recognized Organization with the Audit requested by the Flag State prior to departing the port.
