The Master's Panamanian STCW II/2 credential had an expiration date of 10 May 2017. On 30 November 2017, the company submitted his expired license to Vanuatu Maritime Affairs to obtain a Flag State endorsement. Vanuatu Maritime Affairs provided a confirmation receipt of application granting him the allowance to sail on board a Vanuatu Flag vessels for a 90 day grace period even-though the submitted license was 6 month passed it's expiration date when the application was submitted and received by the Flag Administration. The vessel's Master signed on board 18 JUN 2017 and the company contrary with STCW I/14 failed to verify that the master had a valid license. The company submitted an application for an endorsement on 30 NOV 2017 on behalf of the Master, who did not have a valid license and who had been operating on the vessel for five months and therefore recommend an external audit. 74 SOLAS(14) IX/3.1 ISM CODE 6.2 30(b)(c)

An existing ship that cannot maintain compliance with the provisions of this Chapter shall not be assigned Restricted Area III and Unrestricted by the Administration. The Recognized Organization issued the vessel a certificate with an endorsement for unrestricted service despite the absence of a required fire suppression system or a steering gear electrical power failure alarm. CCSSC 3.1.2.

When ever an accident occurs to a ship or a defect is discovered a request should be made immediately to the Administration responsible for issuing the relevant certificate for a survey as may be required by 1.10 be carried out as soon as practicable. Port engine saft coupling bolts connecting to transmission were sheered while maneuvering causing engine to loose propulsion. CCSSC CH. 1.11.3 60bc
