The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. Portside main diesel engine and generators fuel shut off valves were found inoperable due to a copper piping was preventing its operation by blocking its closure caused by a negligent operation. 46 CFR 182.455(4)/ 46 CFR 176.830 (a).

Boundaries that separate machinery spaces from accommodation spaces, service spaces, and control spaces must be lined with noncombustible panels or insulation approved in accordance with § 164.009. A small segment of the non-combustible panels was found deteriorated in the portside engine room, below the engine access hatch. 46 CFR 177.410 (4).

All observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. The portside main diesel engine oil breather hose was found leaking, with a paper towel acting as a plug, and leaking oil on top of the battery, posing a fire risk. 46 CR 176.830.

The equipment required for lifefloats and buoyant apparatus must meet the following specifications: Each paddle must be of at least 4 feet in length, lashed to the life float to which it belongs and buoyant. All 06 paddles were found fractured and deteriorated. 46 CFR 180.175 (f)(2).

A vessel of more than 65 feet in length must carry a minimum of three life buoys of not less than 24 inches in diameter. 02 lifering buoys were found with the grab-line deteriorated and faded.46 CFR 180.70 (a)(3).

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility may be required to perform the required inspections, maintenance procedures, and hydrostatic pressure tests. Provide a current pre-engineered fixed fire extinguishing system servicing report in compliance with NFPA 10 standard from a qualified servicing facility. 46 CFR 176.810 (2).

All observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Properly repair dead-ending wires under the operating station.46 CFR 176.830.

Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions. Starter failed while vessel was moored. 46 CFR 183.200(a)

Electrical wiring was found penetrating the void 2 watertight bulkheads (fwd and aft), Properly seal to make watertight and verify all other voids bulkheads. 46CFR176.600(d) Code 17C.

Where a flexible fill pipe section is necessary, suitable flexible tubing or hose having high resistance to salt water, petroleum oils, heat and vibration, may be used. Such hose must overlap metallic pipe ends at the least 11/2 times the pipe diameter and must be secured at each end by clamps. The flexible section must be accessible and as near the upper end of the fill pipe as practicable. When the flexible section is a nonconductor of electricity, the metallic sections of the fill pipe separated thereby must be joined by a conductor for protection against generation of a static charge when filling with fuel. Connect conductor to port and starboard fuel fills prior to fueling to the satisfaction of the marine inspector. 46 CFR 182.445 (g) 16(c)

A penetration in a watertight bulkhead (WTB) must be as high and as far inboard in the bulkhead as practicable, and made watertight. Both WTBs in FWD engine compartments have several screw holes from previously installed equipment (fans) in engine compartment. 11 holes in STBD, 9 holes in port side.

A vessel must comply with the structural design requirements of the standards listed for the hull material of the vessel. Upper 2nd transverse frame in port FWD compartment is dislodged from tabbing mounting frame to longitudinal frame.

A vessel must be fitted with a FCC type accepted general marine radar system for surface navigation with a radar screen mounted at the primary operating station. Radar system on board is not operational. Vessel restricted to 49 passengers. 46CFR184.404.

One ship station license will be granted for operation of all maritime services transmitting equipment on board a vessel. Vessel’s FCC Bridge to Bridge and Safety Radiotelephony Certificates have call signs that do not match the FCC Station License. 47CFR80.13.

A vessel of more than 65 feet in length must carry a minimum of three life buoys of not less than 24 inches in diameter. Port aft life buoy grab line is wasted and requires replacement. Starboard aft life buoy drain plug is wasted and requires replacement. 46CFR180.70(a)(3).

Boundaries that separate machinery spaces from accommodation spaces must be lined with noncombustible panels or insulation approved IAW 164.009. Port aft engine compartment overhead fire insulation is falling down and requires repair.46CFR177.414.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew from electrical hazard. Operating station window allows water intrusion to drip down on electrical equipment and requires resealing.46 CFR 183.200.

A hatch in a cabin top must be weathertight. An escape hatch in overhead of passenger accommodation near bar areas allows rainwater to leak into the passenger accommodation and requires resealing. 46CFR179.360 (a) (3).

Each window and its means of attachment to the hull must be capable of withstanding the maximum load from wave and wind conditions. The port and starboard forward most windows in the passenger accommodation allows rainwater intrusion into passenger accommodation and requires resealing. 46CFR177.1020.

A suitable metal marine type strainer and strainer bowl, must be fitted in the fuel supply line in the engine compartment. Port and starboard machinery space fuel filters were missing strainer bowls.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by electrical equipment. #1 Engine starting battery had exposed wiring near its positive terminal.

The number of penetrations in a watertight bulkhead must be minimized and watertight. Port void, bunker compartment was missing packing in the forward bulkhead penetration for wiring/cabling.

Electrical equipment exposed to weather or corrosive environments must be watertight, of suitable construction and corrosion-resistant. Aft auxiliary steering operating station has opening on panel that exposes electrical wiring to a wet corrosive environment. Owner/operator is required to close and maintain as drip-proof for spaces containing electrical equipment.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment. The port side aft bilge suction was inoperable when conducting a test.

Spanner wrenches must be provided where a 40mm (1.5") diameter firehose is required. The vessel did not have a spanner wrench at the firehose station.

PORT main is not functioning. USCG needs to witness repairs after cause of the problem is discovered. vessel is not authorized to operate until clear by USCG Marine Inspector.

VESSEL IS REQUIRED TO HAVE 02 VHF RADIOS INSTALLED. 1 VHF RADIO IS REQUIRED BY FCC AND THE SECOND VHF RADIO IS REQUIRED BY THE BRIDGE-TO-BRIDGE ACT. DURING EXAM IT WAS OBSERVED THAT THE VESSEL HAS ONLY ONE VHF RADIO INSTALLED. INSTALL ONE MORE VHF RADIO AS REQUIRED BY A CERTIFIED FCC TECHNICIAN.

01 ring life buoy is missing its labeling

All three (03) life floats need new labeling.

Make each lifefloat attached to the painter individually.

Vessel is restricted to operating in daylight hours only until the masthead light is properly installed, IAW Seventh Coast Guard District Certificate of Alternative Compliance and to the satisfaction of the attending Marine Inspector, at a height of 4.21 meters (13.8ft) above the main deck and no less than 2.0 meters (6.6ft) above the side lights.

Vessel draft or loading marks are missing from hull. Provide hull marking in accordance with 46 CFR 185.602.

Provide MSC approved plan for 220/110 volt Alternating Current system. Generator usage prohibited while passengers are onboard.
