The owner or operator of an OSV shall make the vessel available for inspection by a marine inspector each time an accident occurs, or a defect is discovered that affects the safety of the vessel; or the effectiveness or completeness of its lifesaving, fire-fighting, or other equipment. The vessel experienced a failure of the port main engine due to #4 cylinder head overheating. Vessel will return to Port Fourchon to undergo repairs prior to departure.

No person shall conduct an oil or hazardous material transfer unless a drain is closed by mechanical means. Small discharge containments around hose connection/vents on both port and starboard sides were missing or provided with non-mechanical means for closure. Mechanical means to close containment drains were obtained and put into place. 33 CFR 156.120

Electric installations on vessels must ensure: Protection of passengers, crew, other persons, and the vessel from electrical hazards. Two dead end cable in tank farm overhead; STBD main diesel engine open per-heater control box; Faulty receptacle on STBD deck cargo rail.

A main transverse watertight bulkhead may not have non-watertight penetrations below the bulkhead deck. Non-watertight penetration identified behind port MDE.

Equipment in support of the normal operation of the emergency power source includes, but is not limited to sound powered phones. E-generator room has a deteriorated sound powered phone cable. 46 CFR 112.05-5 (d)

If the unloading of a substance of category Y or Z is not carried out in accordance with the Manual, a prewash shall be carried out before the ship leaves the port of unloading, unless alternative measures are taken. The vessel has consistently offloaded drilling mud and stated in their cargo record book that tank stripping did not occur. The Cargo Record Book instructs the master when tank cleaning occurs labeled under section E that a receipt must be obtained. There are no receipts on board of discharges of liquid mud residue cleaning to shore side facilities. MARPOL Annex II Reg. 13.7.1.2

Maintain a ballast water management (BWM) plan that has been developed specifically for the vessel and that will allow those responsible for the plan’s implementation to understand and follow the vessel’s BWM strategy and comply with the requirements of this subpart. The BWM plan states that a specific form shall be used and maintained for 2 years of each ballast water operation, however the crew is not using the provided form and not following the ship specific plan. 33 CFR 151.2050(g)

Machinery spaces of category A above 500m3 in volume shall, in addition to the fixed fire-extinguishing system requited in paragraph 5.1.1, be protected by an approved type of fixed water-basted or equivalent local application fire-extinguishing system, based on the guidelines developed by the organization. MSC.1/Circ. 1387 section 3.1.6 states “operating instructions for the system should be displayed at each operating position.” The fire control plan on board states the valves outside of the machinery space located in the passageway of the tank farm is a “remote release station” for water mist in the engine room. The instructions are not clear on how to operate the system and the control for the pump starting is located in the forward bow thruster room.

Fire detection and fire alarm systems shall be kept in good working order. Fire detection panel shows a fault for the port engine smoke detector.

All lifesaving appliances shall be in working order and ready for immediate use. The rescue boat engine would not start. 74SOLAS(20) III/20.2

Any ship of 400 gross tons and above but less than 10000 gross tons shall be fitted with oil filtering equipment. The oily water seperator was inoperable. 705 - Vsl shall not sail international unitl OWS is operable and IOPP valid.

Each embarkation station must be adequately illuminated by lighting supplied from the emergency source of electrical power. Observed two embarkation station lights out on the port side on e-power.

The means of control of any fixed gas fire extinguishing system shall be ready accessible and operable. The master shall ensure the appropriate tests and inspections of fire extinguishing equipment. Observed emergency generator room fixed CO2 control cylinder that is in need of recharge of control gas. (FSS Code 5.2.1.3.3 & 46 CFR 132.350(a))

Fire doors on 01 deck did not self-close as required by 74 SOLAS 14 (Consolidated) Chapter II-2/9.4.1.1.5.1
