Vent outlets from all tanks which may emit flammable or combustible vapors, such as bilge slop tanks and contaminated drain tanks, must be fitted with a single screen of corrosion-resistant wire of at least 30 by 30 mesh, or two screens of at least 20 by 20 mesh spaced not less than one-half inch (13mm) nor more than 11/2 inches (38mm) apart. Vessel had two fuel vents with compromised wire mesh.

Electrical equipment on a vessel must be designed, installed, and maintained to provide services necessary for safety under normal and emergency conditions, protect crew and the vessel from electrical hazards, and prevent electrical ignition of flammable vapors. Smoke and overheating on the port generator was reported due to possible bearing failure. Vessel may transit to Port Fourchon for diagnosis and repair(s). Vessel may not depart Port Fourchon until repairs have been completed to the satisfaction of USCG.

All fuel oil piping subject to internal head pressure from a tank must be fitted with positive shutoff valves at the tank. Valves must be capable of being actuated remotely. Valves for STBD #1, 2, and 3 storage tanks, and PORT day tank and FOP were inoperable at time of inspection. Expanded exam of SMS revealed vessel's SMS does not include inspection and maintenance of remote actuated fuel valves.
