Fire fighter's outfits shall comply with the fire systems code. Personal equipment shall consist of the following: protective clothing of material to protect the skin from the heat radiating from the fire and from burns and scalding by steam. The outer surface shall be water resistant. The PSCO found that both firefighting suits were torn and foil was peeling.

Personnel responsible for the operation of an incinerator installed in accordance with the requirements of paragraph 6.1 of this regulation shall be trained to implement the guidance provided in the manufacturer's operating manual as required by paragraph 7 of this regulation. The refractory in the main burn chamber of the incinerator had a large crack running through the top.
