Resolution: Received Class Report. the O-rings for water jackets on Cylinders #1 and #2 were renewed by ship's crew. After completion of repairs the ME has been examined by BV surveyor satisfactory and no apparent leakages were noted.

PSCO observed document [withheld] pages 3 and 4 before arrival/departure at a port the master shall check the preparation and proper functioning of the ship's equipment in accordance with the departure checklist. Master did not meet the requirements and the equipment in deficiency 001 was not functioning properly. Required ISM audit from certificate issuing authority within 3 months. 74 SOLAS (09) IX/3.1, ISM 5.1.1

PSC OFFICERS OBSERVED AN OIL SHEEN AND AIR BUBBLES AT THE STERN OF THE VESSEL AND MODIFICATIONS TO THE STERN TUBE SEAL SYSTEM. THE VESSEL WAS DISCHARGING OIL INTO THE WATER DIRECTLY ABOVE THE STERN TUBE. MARPOL ANNEX I/15.2 30ABC, 60ABC
