Two sets of steel ladders as widely seperated as possible leading to doors in the upper part of the space similarly seperated and from which access is provided to the open deck in general, one of these ladders shall provide continuous fire shelter from the lower part of the space. The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Fire extinguishing appliances shall be kept in good order and be available for immediate use at all times. PSCO's observed 43 out of 54 total CO2 cylinders with cotter pins installed at the top-valve location on the cylinder head. Per the manufacturer's instructions, the presence of the cotter pins renders the cylinders inoperable. the cotter pins were installed into the top-valves on 22MAY2024 as a safety precaution while conducting cleaning, and the pins were inadvertently not removed, rendering the system not ready for immediate use for a total of 22 days. 74 SOLAS (97) Ch II-2/21

Where the emergency source of electrical powe is a generator, it shall be started automatically upon failure of the main source. PSCO OBSERVED CLASS "A" FIRE DOORS LOCATED IN THE FORECASTLE, STEERING GEAR ROOM AND GALLEY, INDICATED AS SELF CLOSING ON THE FIRE CONTROL PLAN, SECURED OPEN USING HOLD BACKS AND WOODEN WEDGES. PSCO OBSERVED 20 FIRE EXTINGUISHERS NOT SERVICED IN ACCORDANCE WITH ONBOARD PROCEDURES. PSCO OBSERVED 20 TOTAL SPARE FIRE EXTINGUISHERS LOCATED IN THE AFT STORAGE COMPARTMENT, 19 HAD NOT BEEN INSPECTED SINCE FEBRUARY 2018. FIRE EXTINGUISHERS LOCATED IN THE EMERGENCY GENERATOR COMPARTMENT, ENGINE ROOM LOWER LEVEL ON THE STARBOARD SIDE, AND ENGINEN ROOM NEAR THE FUEL OIL AND LUBE OIL PURIFIERS HAD NOT BEEN INSPECTED IN THE MONTH OF APRIL. ADDITIONALLY, THE EXTINGUISHER LOCATED IN THE RESCUE BOAT HAD NOT BEEN INSPECTED SINCE DECEMBER 2017. PSCO OBSERVED 02 FIRE HOSE BOXES ON THE PORT SIDE TIED SHUT WITH NYLON LINE THAT TOOK APPROXIMATELY 03 MINUTES TO UNTIE, WHICH GAINS ACCESS TO THE FIRE HOSE, FIRE HOSE NOZZLE, AND SPANNER WRENCH.
