The purpose of this regulation is to maintain and monitor the effectiveness of the fire safety measures the ship is provided with. For this purpose, the following functional requirements shall be met fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. PSCO observed deteriorated pilot hoses on all of the Co2 cylinders for the engine room. 74 SOLAS II-2/14.1.1

To assist in casualty investigations, ships, when engaged on international voyages, subject to the provisions of regulation 1.4, shall be fitted with a Voyage Data Recorder (VDR); the VDR should continuously maintain sequential records of pre-selected data items relating to the status and output of the ship's equipent and command and control of the ship. (74 SOLAS (14) V/20.1)(Resolution A.861 (20)) While on ship's bridge, PSCO observed an error message on the VDR; after restarting the VDR, the error message persisted. Error message said "Communication with the Final Recording MEdium has been lost."

2565 - The Company should establish and maintain procedures to control all documents and data which are relevant to the safety management system. The company should ensure that obsolete documents are properly removed. Vessel uses software program Shipmanager for SMS documents. Updated to SMS documents are pushed from shore to ship and marked as "not read" within system. After reviewing an updated document, crew must click button in software program acknowledge receipt and review and change status to "read". PSCOs observed that over 50 % of SMS documents showed as "not read". This presents objective evidence that the most current version of documents are not being implemented on board. As an example, upon request the ship's crew presented version 01.01.02 dated 16NOV2015 of Form F01015 (Critical Equipment List). However, per Shipmanager, version 01.01.03 dated 30JUN2016 is the most recent. Provide documentary evidence that all updates to the SMS have been read onboard and properly implemented. 74 SOLAS 14 IX/3.1 REF ISM 11 15ac

2055 - Maintenance, testing, and inspections of firefighting equipment shall be carried out based on guidelines developed by the Organization [MSC Circ 1432] and incorporated into the firefighting maintenance plan. Ship's firefighting maintenance plan does not substantially correspond with the requirements of MSC Circ 1432 and Antigua and Barbuda (flag state) Circular 2012-011 requirements. For example, items in PMS for monthly tests of fixed CO2 fire extinguishing system do not correspond with requirements, and multiple items for annual testing of main mains/pumps are missing. 2550 - The inspections mentioned in 10.2 [periodical inspections of equipment] as well as the measures referred to in 10.3 [measures aimed at promoting the reliability of critical equipment] should be integrated into the ship's operational maintenance routine. The ship's critical equipment list (Form F01015) lists the flexible coupling between the ME and gearbox as a piece of critical equipment, and stipulates that the periodical maintenance required is a monthly visual check. However, this PMS item is only required every 3 months per the vessel's electronic PMS schedule in Shipmanager (task C-003-C-GB). Thus, the two periodicity requirements do not align. Additionally, F01015 lists quick closing valves as critical equipment. However, there are only PMS tasks for FO/DO valves, but none for lube oil, within Shipmanager. Company shall review critical equipment list and compare against PMS line items in Shipmanager to ensure consistency and verify that periodical maintenance & checks are being conducted. 74 SOLAS 14 IX/3.1 REF ISM 10.4 15ac

0725 - A ship when in port of another contracting government is subject to control by officers duly authorized by such government concerning operational requirements in respect of safety of ships, when there are clear grounds for believing that the master or crew are not familiar with essential shipboard procedures relating to safety of ships. Vessel is classed as automated-unmanned machinery space. The PSCO observed the Minimax hyper mist system discharge outlet valves to be in the manually closed position, rendering the system incapable of fighting a fire in high risk fire areas in the engine room. 74SOLAS(14)XI-1/4.1 and 74SOLAS(14)II-2/2.1.2 30ABC

Resolution: PSCOs boarded vessel and verified that PMS line items for weekly, monthly, and yearly checks of water-spray system had been added into vessel's SMS (Shipmanager program). Received and reviewed copy of external ISM audit by RO (DNV-GL).
