Before the ship leaves port and at all times during the votage, all life-saving appliances shall be in working order and ready for immediate use. While at outer anchorage at the Los Angeles/Long Beach, vessel attempted to lower the free fall lifeboat to maneuver in water for it's quarterly requirement. Upon entering the water and still attached to the bridal cable, the lifeboat made contact with the ship's rudder 03 times causing 02 penatrations through the lifeboat's stern hull and damage to be seized against it.
Action required: 15 - Rectify deficiencies by next port
