All ships 500GT and upwards shall have rudder, thrust, pitch and operational mode indicators to determine and display rudder angle. PSCO observed inaccurate readings from bridge rudder angle repeaters when turning steering gear beyond 024 to starboard. 74 SOLAS 2020/CH V/19.2.5.4

The machinery shall be of a design and construction adequate for the service for which they are intended. It was reported that the vessel had experienced propulsion irregularity due to M/E air starting valve limit switch was found loose due to vibration. Crew re-tightened the limit switch at correct position and was found in good working order at this time. SOLAS 74(2014) II-1/26

The Company should establish procedures, plans and instructions, including checklist as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. PSCO observed during the Damage Survey that the Engine department crew did not fill out the pre-departure checklist per the vessel Safety Management System. Chief engineer did not ensure that the pre-departure checklist was not filled out before departure. The company shall provide corrective action plan ensuring pre-departure checklist is filled out every departure, with the concurrence of the vessel Administration or the Certificate issuing authority. Resolution: Class approved the company's newly established checklists for pre-arrivals as well as conducted a shipboard safety meeting with the engine room crew discussing the implementation of the checklists. The company will also verify checklists are conducted.
