Oil fuel lines shall be screened or otherwise suitably protected to avoid oils spray or oil leakage onto the sources of ignition. Marine inspectors identified multiple high pressure fuel lines on machinery without shielding. SOLAS II-2,Part B, Reg. 4, Par. 2.2.5.3

The periphery of the landing area should be delineated by omni-directional green perimeter lights visible from on or above the landing area; one green perimeter light was burnt out on the flight deck. Must be fixed prior to flight operations or within 30 days.

No pressure vessel need be hydrostatically tested except when a defect is found that, in the marine inspector's opinion, may affect the safety of the pressure vessel. In this case, the pressure vessel should be hydrostatically tested at a pressure of 1 1/2 times the maximum allowable working pressure. Marine Inspectors identified a pressure vessel on the 01 aft deck that has not been tested, utilized for the water sprinkler system. Could not verify structural integrity and needs to be tested to 1.5 MAWP and subsequently 1.25 MAWP twice in a 5 year period. 46CFR 61.10-5 (b)(3);(d)

A vessel of 20 meters or more in length shall be provided with a bell in addition to a whistle. Marine inspectors identified no mountable location for utilization of the ships bell. 33CFR 83.33

Each vessel must be fitted with a compass suitable for the intended service of the vessel. Except aboard a vessel limited to daytime operation, the compass must be illuminated. Marine inspectors identified the compass had no illumination. Repair within 14 day or before night time operation. 46CFR 130.340

A minimum of two two-way portable radiotelephone apparatus for each fire party for fire fighter's communication shall be carried on board. Marine Inspectors identified 01 radio with a dead battery, and 02 suits with flashlights not operating with full brightness. SOLAS II-2 10 LSA 3 2.1.1.4

Each abandon ship drill shall include: checking that life jackets are correctly donned. During the abandon ship drill crew members donned non-SOLAS approved life jackets. SOLAS 3.4.1.4

Liftboat jacking systems, liftboat legs, liftboat leg pads, and arrangements for supply of water to firemains, as well as the items listed by 126.430 of this subchapter, will normally be inspected during an inspection for certification and periodic inspection. Make the vessel available inspection by a marine inspector for all items listed in the "Five (5) Year Drydock Inspection Items" found in enclosure (2) to CG-543 Policy Letter 07-02 as discussed in the email dated 22APR2024. 46 CFR 134.120

The hull above the port leg pad showed multiple signs of cracked welds. There had been previous repairs made to these areas that continue to crack without any damage to the surrounding steel. Each crack appears to be in the location of an interior bulkhead. Make the spaces available for a more detailed inspection. Additionally provide a repair proposal that includes preventative measures accounting for crack welds occurring with no damage to the surrounding areas. 46 CFR 126.150(a)

This part contains requirements for the design, construction, and installation of electrical equipment and systems including power sources, lighting, motors, miscellaneous equipment, and safety systems. Power was not connected to the portable accommodation modules and the inside of each module was unable to be inspected adequately. Additionally, no safety system is operable without power. The modules shall not be used until they properly inspected and deemed fit for occupation.

Each OSV must be provided with means for testing each visible and audible alarm. Vessel was unable to check low lube oil pressure as per approved ABS PSTP's due to rep not having proper testing equipment.

Several cracks were noted in the overhead (main deck) of the port and stbd z drive rooms.

Side shell IWO port fuel tank and port Potable water tank was damage by tug while assisting vessel into the dry dock. Inserts are required in both tanks to include internal framing repair.

All structural fire-protection materials must be approved by the Coast Guard in accordance with the regulations of subpart 2.75 of this chapter. Vessel needs to replace structural fire protection A-60 insulation in way of the port engine room supply ventilation.

E-lighting at embarkation stations not operable.

International shore connections P/S has valves that are inoperable and missing washers (8) and gaskets.

Starboard aft leg was found with a fractured weld at the pad to can weld seam (water found inside). Gouge and re-weld effected weld seam. Five areas along the overhead to aft bulkhead connection within the spud mat pocket were fractured along the weld seam. Provide NDT report showing satisfactory repair IAW approved weld procedure.
