Resolution: Company replaced entire prime mover. Conducted visual inspection and tested emergency shut down.
Manholes and flush scuttles in position 1 or 2 or within superstructures other than enclosed superstructures shall be closed by substantial covers capable of being made watertight.
#1 STBD forward hatch coaming and #2 STBD forward hatch cover found severely deteriorated, diminishing watertightness. In addition, STBD bow rake hatch missing dog due to deteriorated hinge.
A one time cargo discharge is authorized to allow required preparations for welding.

Scantlings, material, and workmanship, subdivision of cargo spaces, fitting of cofferdams, and testing of tanks shall be at least equivalent to the requirements of the American Bureau of Shipping.
Require gauging on indicated locations of diminished scantling on cargo tank trunk and STBD bow deck.

Disks or disk faces, seats, stems and other wearing parts of valves shall be made of material possessing corrosion and heat resisting qualities suitable for service conditions to which they may be subjected.
Marine inspectors observed extreme corrosion on the reach rod stem pipes for the master suction, cargo 1, cargo 2, and deep well stripping valves located on the open deck of the vessel. Vessel is authorized one time discharge within the port of Houston. Vessel shall be gas-freed prior to conducting permanent repairs. Repairs shall be made to the satisfaction of the Coast Guard Marine Inspector.
