An owner or operator of a ship to which this subpart applies must review the shipboard oil pollution emergency plan annually and submit a letter to Commandant (CG-MER) certifying that the review has been completed. This review must occur within 1 month of the anniversary date of Coast Guard approval of the plan. SOPEP was found expired.

No repairs or alterations affecting the safety of the vessel with regard to the hull machinery, or equipment shall be made without the knowledge of the Officer in Charge, Marine Inspection. Hull insert between Frame 6.25-6.5 chine edge; ¾” A36 approx. 18”x18.” 4. Frame inserts (3) at Frames 4.5, 4.75, and 5.75 just below S3; 3/8” A36 8”x28.” Ballast Tank 1 Port: 1. Long B19 Port: Inset against hull between Frame 4 and 4.25 3/8” A36, approx. 24” x 10.” 2. During each inspection or re-inspection for certification, all wing voids, rakes, cofferdams, and other void spaces on barges and checked from on-deck for the presence of water or cargo indicating hull damage or cargo tank leakage. Open aft peak center void manhole cover for inspection. 46 CFR 91.40-3 (b)

The spring in a relief valve in service for pressures up to and including 250 pounds per square inch shall not be reset for any pressure more than 10 percent above or 10 percent below that for which the relief valve is marked. Barge has relief valve on pressure vessel that was marked for 150 psi and relieved at 125 psi. Retest with calibrated pressure gauge or replace with bench tested valve. 46 CFR 54.15-10 (g)

Manholes in position 1 or 2 shall be closed by substantial covers capable of being made watertight. Unless secured by closely spaced bolts, the covers shall be permanently attached. Void 3P has cover with 2 studs broken off and 3 nuts missing. 46 CFR 42.15-40 (a)

At each inspection for certification and periodic inspection the marine inspector shall conduct such tests and inspections of the main propulsion and auxiliary machinery and of its associated equipment as he/she feels necessary to check safe operation. Barges second generator unavailable for testing. Schedule marine inspector for testing. 46 CFR 61.20-3

DEMONSTRATE OVERSPEED TRIP FOR BOTH DIESEL GENERATORS. EACH GENERATOR PRIME MOVER MUST HAVE AN OVERSPEED DEVICE THAT IS INDEPENDENT OF THE NORMAL OPERATING GOVERNOR AND ADJUSTED SO THAT THE SPEED CANNOT EXCEED THE MAXIMUM RATED SPEED BY MORE THAN 15 PERCENT.

VESSEL'S SECURITY PLAN DOES NOT REFLECT CURRENT VESSEL OPERATION AND OWNERSHIP. THE COMPANY SECURITY OFFICER MUST ENSURE A VESSEL SECURITY PLAN IS DEVELOPED AND IMPLEMENTED FOR EACH VESSEL.
