General arrangement plans shall be permanently exhibited for the guidance of the ship's officers, showing clearly for each deck the control stations, the various fire sections enclosed by "A" class divisions, the sections enclosed by "B" class divisions together with particulars of the fire detection and fire alarm systems, the sprinkler installation, the fire-extinguishing appliances, means of access to different compartments, decks, etc., and the ventilating system, including particulars of the fan control positions, the position of dampers and identification numbers of the ventilating fans serving each section. Vessel fire plan did not match observed arrangements on the vessel. Vessel is to work with class to rectify fire control plan.

Machinery spaces of category A containing internal combustion machinery shall be provided with one of the fixd fire extinguishing systems in paragraph 4.1. The insulation of a deck or bulkhead shall be carried past the penetration, intersection or terminal point for a distance of at least 450 mm in the case of steel or aluminum structures. The insulation on the port side of the reverse osmosis room was torn out. This lagging is required to be repaired.

The initial survey shall include a complete inspection of the structure of pressure vessels and their appurtenances. Three pressure vessels on board were found to not have data tags on their relief valves. Install relief valves containing their data tags.

Provisions shall be made to prevent overpressure in any part of the oil fuel system. The air vent on the fuel transfer station was leaking and did not have a vent cover or flame screen. A vent cover is required and no fuel should be permitted to leak from the vent.

A watertight door may be opened during navigation to permit the passage of passengers or crew, or when work in the immediate vicinity of the door necessitates being opened. The door must be closed when transit through the door is complete. Three doors were found propped open with door indicators taped closed. All doors must be closed with indicators operating.

The port-side escape hatch leading to the Z-Drive had water on the and light was penetrating through the gasket. In addition, there were two penetration plugs that were missing.

Non-metallic flexible hose may be used in lube oil, fuel oil, and fluid power systems only where flexibility is required and in lengths not exceeding 30 inches. There was a fuel hose that was connected to the fuel transfer station that was approximately 6-8 feet in length.

All electric apparatus must be designed, constructed, and installed to prevent any person from accidentally contacting energized parts. On the deck near the rescue boat, there was a penetration that was covered in foam as well as exposed wiring that leads down to the equipment room.
