Objective evidence discovered during an expanded ISM exam revealed the following non-conformities: The vessel failed to fully implement the requirements of the ISM Code through their Safety Management System (SMS) procedures as evident by the following deficiencies indicating that the vessel and/or company are not meeting the SMS requirements. Recommend an external audit. The company should established procedures, plans and instructions including checklists as appropriate, for key shipboard operations concerning safety of the personnel, ship and protection of the environment. The vessel’s SMS does not contain any checklists or procedures regarding material conditions relating to fire hazards in the engine room. SOLAS 74 (2014 Cons.) IX/3.1 ISM Code Part A

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. While PSCO was conducting round of engine room he observed a clear liquid that was identified as diesel fuel oil coming from a 6 inch fracture in the #2 port MGO storage tank. SOLAS 74 (2014 Cons.) I/14(a)

Resolution: Temporary repaired crack with epoxy plastic. PSCO verified repair and vessel stated they would not fill tank within 2 Meters of crack to prevent spills until permanent repairs can be made.

Resolution: Vessel engineers cleaned effected areas. Determined locations of leaks and conducted required maintenance to prevent Fuel/Oil leaks. Vessel will maintain clean bilges and prevent the risk of fire from fuel and lube oil sitting in the bilge.

In spaces where penetration of oil products is possible, the surface of insulation shall be impervious to oil or oil vapors. Due to a fracture discharging diesel from the #2 MGO storage tank, the adjacent pipe became saturated covering 25-30 feet of lagging, creating a fire risk. SOLAS 74 (2014 Cons.) II-2/4.3

Resolution: Saturated lagging was replaced with new lagging.

PSCO observed two areas of oil soaked lagging of the heavy fuel oil piping in the purifier room and adjacent machinery space, indicating potential leaks in the piping system. SOLAS 74 (2014 Cons.) II-2/4.3

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. The design shall have regard to materials used in construction, the purpose for which the equipment is intended, the working conditions to which it will be subjected and the environmental conditions on board. PSCO observed an active oil leak coming from loose hardware on the #1 crankcase access cover creating a major fire safety hazard. SOLAS 74 (2014 Cons.) II-1/26.1

Resolved 22 February 2019
Resolution: Crankcase access cover hardware tightened and verified. No leaks noted and standing lube oil was cleaned from the bilges.

Every lifebuoy shall if it is intended to operate the quick-release arrangement provided for the self-activated smoke signals and self-igniting lights, have a mass of not less than 4kg. PSCO discovered starboard bridge wing quick-release arrangements with a 2.5 kg lifebuoy. Crew replaced with a 4.3 kg lifebuoy. SOLAS 74 (2014 Cons.) III/7.1.1 LSA Code 2.1.1.7
