74 SOLAS (04) III/31.1.2.2- In lieu of paragraph 1.1, cargo ships may carry one or more inflatable or rigid liferafts, on each side of the ship, of such aggregate capacity as will accommodate the total number of persons on board. Port side life rafts spontaneously inflated and fell in water. Vessel does not have enough life rafts to meet total capacity on either side, or to meet what is required on their certificate.

Vessel reported that their radar was inoperable leaving Sector Delaware Bay. Repairs to be made in port.

Each vessel of 10,000 gross tons or more must have, in addition to the radar system under 164.25(a), a second marine radar system that operates indepenently of the first. Vessel's S-Band radar is inoperable.

Guard rails or bulwarks shall be fitted around all exposed cargo decks. Cargo handling platforms were found to be missing fall protection. ICLL 66 25 (2) 50A

General arrangement plans shall be permenantly exhibited for the guidance of the ship's officers, showing clearly each deck, the control station, the various fire sections enclosed by "A" class divisions, "B" class divisions, togeter with particulars of the fire detection and fire alarm systems. The doors to the cable and locker space on the poop deck were not fitted with the proper "A" or "B" class door as required on the fire control plan. 74SOLAS (2004 CONS) II-2/15.2.4.1 50A

30ac- The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. In meeting these requirements, the company should ensure that appropriate corrective action is taken. The company's protracted response to requisitions led to a culture of work-arounds in lieu of appropriate corrective measures. Nearly all light fixtures had only one of two bulbs despite a two month old requisition. Repair parts have rendered doors nearly inoperable. The safety management system should include procedures ensuring that non conformities, accidents and hazardous situations are reported to the company, investigated and analyzed with the objective of improving safety and pollution prevention. Five quick closing fuel valves were circumvented rather than serviced for proper operation. Any non-conformity is reported, with its possible cause, if known. All on deck fire-fighting hose boxes had faulty closures and were lashed shut with rope. The company should establish procedures for the implementation of corrective action, including measures intended to prevent recurrence. Two engine room EEBDs were not properly charged despite the vessel having received detainable deficiencies for engine room EEBDs 12 months ago. The SMS should provide for measures ensuring that the company's organization can respond at any time to hazards, accidents and emergencies involving ships. Crew did not produce non Tank Vessel Response Plan for two hours when requested.

Crew members shall be trained to be familiar with the arrangements of the ship as well as the location and operation of any fire-fightingsystems and appliances that they may be called upon to use. During the fire fighting drill: firefighters did not secure SCBA mask to face allowing air to escape; fire hose was tangled in a ball; lifeline handlers did not respond o fire fighters' line commands. The crew was given time to practice fire fighting before performing a second fire drill. Crew demonstrated adequate performance. 74SOLAS(2009) II-2/15.2.2.1

No change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without the sanction of the Administration. The sight glass for the freshwater tank in the steering gear space was leaking at a rate of 1 drop per 5 seconds. A makeshift containment was installed. 74SOLAS(2014) Ch I Reg 1.1b

Every lifeboat shall consist of a food ration totaling not less than 10,000kj for each person the lifeboat is permitted to carry; these rations shall be kept in airtight packaging and stowed in a watertight container. Vessel has only 20 pieces of adequate food rations on board a lifeboat rated for 28. 74SOLAS(2014) Ch III Reg 13.1.4 LSA 4.4.8.12

Protective clothing of material to protect the skin from the heat radiating from the fire and from burns and scalding by steam. During the fire drill, it was observed that the fire team's skin was exposed while wearing the firefighting outfits.

30ac- Oil fill pipes shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space in the event of a fire occurring in the space in which the tanks are situated. 5 quick closing valves have been disabled using fabricated devices or bolts.

Each emergency generating set arranged to be automatically started shall be equipped with starting devices approved by the administration with a stored energy capable of at least three consecutive starts. A second source of energy shall be provided for an additional three starts within 30 min unless manual starting can be demonstrated to be effective. At the time of inspection, the PSCO found only one operable source of stored energy capable of starting the emergency generator. The secondary means employed by the vessel through a manual cranking system was not operable. 74SOLAS (2014) CH II-I / REG 44.2

On all ships, within the machinery spaces, emergency escape breathing devices shall be situated ready for use at easily visible places which can be reached quickly and easily at any time in the event of a fire. It was found that five out of the eleven EEBDs required on the ships fire control plan had no charge. 74SOLAS (2014) CH II-2 / REG 4.3.1

Each hose shall be provided with a nozzle and the necessary coupling. Two fire station hoses were missing nozzles. 74SOLAS (2014) II-2 / REG 10.2.3.2.3.1.1

In meeting the requirements of 10.1, the Company should ensure that: appropriate corrective action is taken. A requisition for the parts to fix the emergency generator was submitted to the company on 04 DEC 2017 and is still outstanding. The five out of eleven empty EEBDs were made aware to the company on 29 NOV 2017 via email from the contractor servicing the vessel’s EEBDs. A requisition for the two missing fire hose nozzles was submitted to the company shortly after the drydock preformed in October 2017. The vessel was allowed by the company to make multiple port calls with no communicated expected delivery date of supplies necessary to repair vital systems. 74 SOLAS (14 Cons)II-2 Reg 19.3.4.2 - 1160 - Suitable wire mesh guards shall be fitted over inlet and otlet ventilation openings. Cargo hold 3 and 4 inlet ventilation openings are deterioted.

74Solas (14Cons) III/Reg32.1.1 - 0650 - Vessels 150 and under 200 meters are requried to have a minimum of 12 lifebuoys. The vessel is carrying 11 lifebuoys.

74 SOLAS (14Cons) 111 Reg.7.1.1 LSA Code 2.1.1.8 Code 0650 Lifebuoys must be fitted with a grabline not less than 9.5 MM in diameter. The grablines on 11 lifebuoys are dry rotted.
