PSCO OBSERVED AN EXCESSIVE WATER LEAK COMING FROM THE #4 CYLINDER HEAD ON THE MDE.

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequae for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to...other hazards.
The vessel experienced an equipment failure while transiting the navigable waters of the United States which adversely affected its maneuverability and posed a hzard to vessels and infrastructure along the Houston Ship Channel.
Submit for review a classification society report to [email withheld] attesting to the cause of the equipment failure, that authorized temporary or permanent repairs have been completed, and the that vessel's full and normal maneuverability has been restored.
