The arrangements for the storage, distribution and utilization of oil used in pressure lubrications systems shall be such as to ensure the safety of the ship and persons on board. PSCO observed main engine experienced safety shutdown in relation to concurrent alpha lube oil alarm.

Resolution: Class tested engine up to slow ahead but could not safely test engine to 70+ RPM. Amended COPT to allow vsl to transit LMR with tugs assists. At 2000 pilot onboard reports engine is consistently running at 90 RPMS. Following pilot’s report, cleared def.
Electrical or other equipment which may constitute a source of ignition of flammable vapors shall not be permitted in these compartments. During exam walkthrough in bosun hold & engine room numerous electrical light fixtures were found compromised and/or inoperative. 74SOLAS(2014) II-1/45.9.2
