AFER ANY SURVEY OF THE SHIP UNDER ARTICLE 14 HAS BEEN COMPLETED, NO CHANGE SHALL BE MADE IN THE STRUCTURE, EQUIPMENT, ARRANGEMENTS, MATERIAL, OR SCANTLINGS COVERED BY THE SURVEY, WITHOUT THE SANCTION OF THE ADMINISTRATION. THE VESSEL SELF REPORTED BENT GIRDERS, CRACKED WELDS, AND OTHER DAMAGE IN WAY OF THE CARGO AREA WHICH HAS AFFECTED THE SEAWORTHINESS OF THE VESSEL.

0725-The PSCO observed the NOVENCO hyper mist system fresh water suction valve in the closed position, rendering the system incapable of fighting a fire in high risk fire areas in the engine room. Vessel is classed for and operates in unmanned mode as per the Engine Log Book. 30abc

According to manufacturer's instructions, the system shall be inspected weekely, monthly, every 6 months, and annually. Crew could not provide record for any of these tests being conducted. 30abc
