Ships of 500 gross tonnage and upwards shall be provided with at least one International Shore Connection. Connection shall be kept onboard together with a gasket, four bolts, four nuts, and eight washers. Port State Control Officer found one connection missing the gasket and the other had bolts that were unserviceable.

The company should identify equipment and technical systems the sudden operational failure of which may result in hazardous situations. The vessel experienced a loss of propulsion that resulted in a grounding within the Houston Ship Channel. This condition posed a hazardous condition within the navigable waters of the United States. Preliminary reports from vessel's crew indicates that a probable cause of the loss of propulsion is an oil mist detector activiating incorrectly; sudden issue with oil mist detector resulted in a hazardous situation. Oil mist detector is not listed on vessel's critical equipment list (Saftey Management System 5 - 5.4). Submit evidence to [email withheld] detailing how this hazardous situation was reported to the company, investigated, and analyzed with the objective of improving safety, including the implementation of corective action and measures intended to prevent recurrence (as detailed and required in ISM A/9).

Resolved 21 February 2018
Resolution: 21FEB18: recieved class report from BV stating they did not attend but accepted picutres and statement from CE stating outstanding deficiency has been rectified. CE had cleaned excess FO and replaceed o-rings/gaskets, tightned fitings. See narative.
