A vessel on a lakes, bays, and sounds, or rivers route must carry three hand smoke distress signals approved in accordance with 160.037. Vessel has 02 hand smoke distress signals instead of the 03 required on board.

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Vessel has 03 5 gallon buckets instead of 2 1/2 gallon buckets.

In a vessel of 12 meters or more in length but less than 50 meters in length: A white, red, green or yellow all-round light, 2 miles. Vessels all around white light was inoperable.

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: (1) Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter) as amended here: (i) Certification or licensing by a state or local jurisdiction as a fire extinguisher servicing agency will be accepted by the Coast Guard as meeting the personnel certification requirements of NFPA 10 for annual maintenance and recharging of extinguishers. The annual maintenance was not conducted for the portable fire extinguishers by the owner or qualified third party.

An adult life jacket must be provided for each person carried on board a vessel. Restricted to 42 passengers until new adult life jackets are on board. 46 CFR 180.71

The manning requirements for a particular vessel are determined by the OCMI after consideration of the applicable laws, the regulations in this part, and all other factors involved, such as: emergency situations, size and type of vessel, installed equipment, etc. Crew CPR/First Aid Certification expired. 46 CFR 15.501

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. Identification of the items may be in words, or the appropriate symbols. Letters and numbers must be at least 2 inches high. Symbols must be at least 4 inches square. Life jacket stowage markings missing and/or incorrect. 46 CFR 185.604

The official number of the vessel, preceded by the abbreviation "NO." must be marked in block-type Arabic numerals not less than three inches in height on some clearly visible interior structural part of the hull. The number must be permanently affixed to the vessel so that alteration, removal, or replacement would be obvious. If the official number is on a separate plate, the plate must be fastened in such a manner that its removal would normally cause some scarring of or damage to the surrounding hull area. Place in service four USCG approved Type I PFDs.

The B-II required by 46 CFR 181.500 and the vessels COI is not in serviceable condition and is not maintaining the appropriate charge as per design. Operator has been required to provide proof of replacement.

The corrosion resistant clip that is required to be attached to the life ring is required to be of corrosion resistant materials. The clip shows signs of excessive wastage and corrosion. Operator is required to provide proof of replacement with required clip.

The owner could not provide proof that 50% of his employees have recieved First Aid/CPR training. Required to provide proof of training.
