Electrical installation in hazardous location shall comply with the recognized standard. MI id'd the following issues in the hazardous areas:Numerous broken bonding straps and loose connectors(i.e. port LNG tank gauge, bunkering area, silencers and FGS ventilation fans, GVU MEGI room, etc). Conduct maintenance and demonstrate proper installation to the attending USCG/ABS. 13102

Gas piping should be protected against mechanical damage and the piping should be capable of assimilating thermal expansion without developing substantial tension. #2 N2 purging piping at the bunkering station is cracked and needs to be repaired. provide a repair plan and welders certificates/procedures to USCG/ABS for review and approval prior to any work being conducted. Once repaired, conduct testing as outlined in the plan to the satisfaction of the attending surveyors/ USCG Marine Inspector. Ref: 13102

All gas piping systems including valves, fittings and associated equipment for handling gas should be tested under normal operating condition before set into normal operation. Fillet weld on vapor gauge line for valve FG717 failed on the LNG vapor piping for LNG tank #2. Follow procedures IAW risk analysis form and proceed to Jacksonville IAW your provided procedures. Provide repair proposal to Sector Jacksonville to be reviewed and approved prior to enacting repairs and completed to the satisfaction of the attending marine inspector.

Changeover to and from gas fuel operations should only be possible at a power level and under conditions where it can be done with acceptable reliability. #3 DFDG fails to operate on LNG and periodically cause blackout of the vessel electrical distribution system (Gas mode only).

All access openings in bulkheads of enclosed superstructures shall be fitted with doors so that the whole structure is of equivalent strength to the unpierced bulkhead and weather tight when closed. Found several WTD knife edges wasted and/or gaskets painted over rendering enclosures non weathertight. Repair/replace and chalk test all knife edges and gaskets between aft accommodation breezeway and the FGS room.

Ships shall be designed, constructed and maintained in compliance with the structural requirements of a class society. Multiple drain pipes, brackets and 440 electrical cable penetrations (unsafe condition) around accommodation space and lifeboat davits were found severely wasted.

A-60 divisions shall be insulated with approved non-combustible materials such that the average temperature of the unexposed side will not rise more than 140deg C above the original temperature. The AC compressor room on "C" deck is missing A-60 insulation.

Precautions against shock, fire and other hazards of electrical origin. Dead grounds were found on 440V Main switchboard and 120V AC Ground Detection panels. Make repairs to the satisfaction to the attending CG Marine Inspectors or ABS Class Surveyor.

Electrical equipment in hazardous location shall be properly installed. CG MI noted the following equipment in zone one hazardous one location improperly installed; Damper Fan Junction in LNG High pressure pup room and several light fixtures stbd side of LNG tank. Covers not secured. To satisfaction of CG MI.

High Pressure CO2 External Indicator (light) is not working. Make proper repairs. Make repairs to the satisfaction to the attending CG Marine Inspectors or ABS Class Surveyor.

Re-stencil Life Ring Buoys, one port side main deck by cargo hold #2 and two on bow. To satisfaction of CG [name withheld] repairs to the satisfaction to the attending CG Marine Inspectors or ABS Class Surveyor.

Make repairs to leaking on Fire Main piping coupler, located stbd side tunnel fwd of WTD. Make repairs to the satisfaction to the attending CG Marine Inspectors or ABS Class Surveyor.

Crew members with direct responsibility for the operation of gas-related equipment on board should receive special training. Sector Jacksonville received notification of a LNG high pressure system liquid leak (Approx 50gals) due improper valve alignment. The following must be completed: 1)Provide a report/email that outlines corrective action to prevent the above issue from happening per the company's SMS. 2)Prior to going back on LNG (high pressure system), the Chief Engineer must ensure proper alignment and provide additional training to crew member directly involve in the operation of the systems. 3)If any cracking/fractures of the affect area are found, Sector Jacksonville must be notification immediately.

Gas detection system should be designed such that when a detector actuates, the vessel operator is able to identify the specific gas detector and its location. Gas detector for the IHM system alarm functions locally, but does not indicates in ECR/CAMS as required.

Electrical equipment fitted within hazardous areas should be in according to the recognized standard. The following equipment in a zone 1 hazardous areas needs to be repaired: Light fixture aft of the starboard LNG tank (fixture not properly secured), and the High pressure pump lubricating oil tank RTD needs to be properly secured.

The ventilation should function at all conditions that the vessel operates. The fire dampers for the GVU (DFDG and MEGI) rooms do not function as designs (does not close properly).

he fire safety objectives are prevent the occurrence of fire and explosion. Marine Inspector found five gallon paint storage in the machinery space near the water mist.

Fire hose must be be connected to the outlets at all times. Marine Inspector noted that the interior fire hoses thru the vessel are found disconnected.

LNG storage tanks for liquid gas should not be filled more than 98% full at the reference temperature. Provide accurate loading/filling capacities for the LNG tanks via official document and/or clarification of the DSEC calculation of the filling limits for the LNG tanks (i.e. bunkering pressure/bars are incorrect as indicated on the chart, etc). Additionally, vessels bunkering procedures need to be updated to reflect the above and current operations (i.e. LNG Fuel Loading indicator, etc).

Troubleshoot, repair and demonstrate proper operation of the vessel USL communication system.

Properly label all newly installed LNG valves(port and starboard tanks) and update the emergency/operations bunkering manual/s drawing as required. Additionally, update CAMS/FGS diagrams/test procedure to reflect the newly installed/re-located LNG valves.

Troubleshoot the UPS units for CAMS and FGS systems to ensure the indicating lights (LED) functions as per design i.e. the bypass light does not work during the testing.

Provide/submit a FEMA,DVTP,schematic or logic diagram that shows the system, including a bill of Materials per MSC letter dated 21 Jan 2021 for the CAMS and FGS Server Migration.

Update DA800D109_4_Fuel Piping Diagram (REV 14) to reflect the removal of FG503 and 603 to include the gauge lines (P&S).

The vessels "A" LNG high pressure pump drive unit failed, causing that part of the system to be inoperable. Provide a detailed proposal and timeline to include re-installation and operational test.

Once MSC has approved the PSPT; shipboard testing of the PSPT to be witnessed by the cognizant Officer of Inspection (OCMI) or their designee prior to the annual inspection for 2021. 01Apr2021

Port side lifeboat has possible damage to fiberglass located on starboard side aft where the boat meets the bumper on the davit. Have authorized service technician evaluate area for possible damage and repair as needed.

The Training Manual shall contain instructions and information on life-saving appliances in the ship. The training manual found in the crew lounges were found to be general in nature and not specific to lifesaving appliances onboard vessel. To be completed to the satisfaction of ABS. Provide documentation of current servicing of nitrogen bottle used for actuation of dry chemical system at LNG bunkering station. MSC.285 (86)3.3.3

Provide documented proof that person responsible to perform inspections & maintenance of electrical installations in explosive hazardous spaces be qualified pursuant to IEC 60079-17. MSC.285(86) 8.3.4. To the satisfaction of the attending marine inspector.

Prove proper operation of uninterrupted power supplies (UPS) for machinery. UPS's are currently in fault condition or in bypass. Make repairs and provide proper operation to the satisfaction of attending marine inspector and class surveyor. SOLAS II-1/4.1

Vessel is prohibited from operating the ME-GI in LNG operation (other than the approved troubleshooting efforts) until the cause/s of methane release being detected the vent riser detector has been idenified and repair.

Repair fracture LNG bunkering piping to the satisfaction of the attending USCG marine inspector.

Troubleshoot, investigate, and determine the cause/s of gas (methane) being detected by the vent mast gas detector of greater than the alarm set points (hi-15%& hi-hi-30% by volume).

Investigate, troubleshoot, and determine the cause/s of the steering gear system rudder drifting to port or/and starboard with no input given, while operating in non-follow up (NFU)mode (underway).

Investigate and determine the cause/s of the presence of gas (Methane), greater than 95% by volume being detected by the vent risere gas detector during bunkering and boil-off (#3DFDG) operations. This detection occurs @ various internal and for different durations.

Trouble shoot and determine the cause of #2DFDG double wall ventilation piping alarm.

Demostrate proper operations of the Fructose remote control communcation system once repaired.

Provide a detailed plan and timeline for removal of the existing FGS Exhaust fan Differential pressure switches.

Provide final copy of the vessels trim and stability booklet to include the general guidance,non-essntial information,supporting cal., simplifying assumption,etc.

Provide documentary proof as to whether or not the Interlink Bunkering communication system shall give an audible alarm @ CAMS or IBF when an ESD is activated. Until the above is determined a watch shall be posted at the CAMS durin bunkering ops.

Properly install STBD watertight doors closing mechanism, 2nd deck tunnel

The vsls interlink communication system does not function as per design i.e. The Fiber Link section of the system does not work when connected to the IBF skid. Additionally, determine wheter or not the system is required to give a audible alarm @ CAMS or IBF when an EDS is activated. Vessel is prohibited from LNG bunkering ops until proper operation of the Interlink comm. system is demo. sat

Troubleshoot and determine the cause of the #2 Steering gear system fault in Non-Follow-up Mode of operation.

Troubleshoot and determine the causes of the Heat Detector Alarms (Including the heat/smoke detector base, starboard side aft, near LNG tank) in the LNG hazarous areas. This issue must be rectified and tested satisfactory to the attending Marine Inspector or ABS. Additional, measures must be employed to properly monitor the LNG areas wntil the above is corrected.

Properly label all emergency lights thru the vessel.

Calibrate/repair #2(stbd)LNG tank TLI (reading incorrectly) to include the hi & hi-hi alarms. Once corrected, demonstrate proper ops. to USCG/ABS

Vent Riser gas detector must be proper installed, calibrated, and tested to the satisfaction of USCG/ABS.

Repair/replace leaking FGS vaporizer gauge/tubing for the DFDG operation and test to the satisfaction of USCG/ABS. Gauge and tubing has been isolated.

Fire and emergenecy fire pump indicators thru the vessel does not function as per design. Ex:Brideg indicator shows the pumps energized while pumps are secured. USCG/ABS

Provide final copies of the operations, emergency and maintenance manuals to the local OCMI for review.

Manual means of the fire dampers closure from inside the FGS DFDG & ME-GI GVU spaces must be proven to the satisfaction of USCG/ABS.

Provide determination as to whether or not following ventilation closures (covers)requires hold back devices for the open position:Port & Stbd "Air Locks"nat' supply vent, N2 generator room vent., and the FGS cabinet room vent.

Vessel's SMS/Deck manuals needs to be amended to reflect the correct % of LEL for the gas detector's installed. Manual's states that the gas detectors alarms @ 30% LEL & Hi-Hi ALARMS @ 60% LEL. the units installed alarms @ 20% LEL and Hi-Hi @ 40 % LEL.

Emergency exit doors and trunk from ECR needs to be clearly marked. USCG/ABS

Properly label all emergency lights thru the vessel. USCG/ABS

Commission and demonstrate proper operation of the Clean Water Separator & OCM to the satisfaction of USCG/ABS.

ME-GI Lube Oil Booster pump leaking and needs to be repaired. USCG/ABS

Fire Hoes and extinguishers in the FGS spaces are not readily accessible and needs to be re-located within the spaces. USCG/ABS

Troubleshoot and repair heat detector that intermittently alarms, LNG area stbd side aft, overhead. Demonstrate proper ops to the satisfation of USCG/ABS

Provide documentary proof of successful commissioning of the Fructose system.

Repair/replace broken and/or non-operable toilets: Bridge Head, E/R head, and QMED's cabin. - USCG

Prove a "FINAL" completed copy of the Vessel's Deck Operating Manual for the local OMCI review.

Troubleshoot and repair fault that's causing the error code for #2 cargo hold smoke detection system as indicated on the panel. Once corrected, demo to USCG/ABS.

The accommodation door located on the STBD side F-deck, leading to the weather deck, latching mechanism is faulty/binding up.

The accommodation door located on the STBD side D-deck, leading to the weather deck, self closing mechanism is not attached.

CO2 room: A section of the A-60 structural fire protection thermal insulation has been removed IVO piping penetrations in the overhead aft area of the room. Replace this insulation.

Install missing flange cover bolt IVO of LNG valve FG758.

Provide original Licenses, STCW, Merchant Mariners Docs. & TWICs for all personnel on the vessel. Cite: STCW 95 I/2, I/10, VI/1, VI/2

Provide proof of USCG approved LNG basic or advance training for crew member [name withheld] shall be billeted to non-emergency response position until completion of approved course. Cite: IMO STCW.7/Circ.23

Conduct emergency Fire drill (including LNG release response) Abandon ship drill, and security drill to the satisfaction of attending Marine Inspector. Cite: SOLAS III/ Reg.19

Ensure bunkering stations dry chemical fire fighting monitor has sufficient locking devise,to facilitate proper position,per approved drawing/manual. Locking devise shall ensure non-movement in both the vertical and horizontal position upon activation of system.

LNG bunkering operations are restricted to No. 1 LNG tank using connection line No. 1 until operational test of line NO.2 has been demonstrated to the satisfaction of the attending ABS surveyor and CG Marine Inspector.

Vessel O/Op shall provide an operational procedures specifying the arrangement of theLNG FGS during initial voyage to Jacksonville. Additionally, O/op shall submit a repair proposal for damaged LNG FGS valves, to ABS and USCG,for approval, prior to conducting repairs.
