Vessel crew ID'd serve cracks IWO the LNG Silencers (MEGI and FGS)bottom weldment and base material. Vessel is prohibited from operating of LNG until the following is received, approved, tested to the satisfaction of the attending Marine Inspector and Class surveyor: - Repair proposal to include weld procedures and welders certificate. - Photos of the cracks and the approved detailed Silencer drawings - NDT reports (Before and after repairs). - Risk assessment relating to the damage. LFF System.

All electrical engineering details and installations shall be designed and installed in accordance with sub-chapter J (Electrical Engineering) of this chapter. Submit repair plan that addresses and corrects deficiencies identified in July 2025 by IDEEA llc, Tote EX_, Hazardous Inspection Document TOTE_EX_INSP_20250711, Rev 0, and to include the broken or disconnected electrical bonding wires not attached on the exterior light fixture aft of the port LNG stowage tank and on IHIM Gas Detector in the FGS Room. IAW 46 CFR 90.15-1, CG-521 Eng Policy letter No. 01-12, IMO MSC 285(86) Reg 4.1. Def Code 13102

Sector Jacksonville received the requested Vibration Analysis report, which reveal that the following valves were outside the acceptable limits of 45 mm/s: FG516, FG616 (N7 piping), FG545, and FG645(N6 piping). A detailed course of action (component supports) must be provide for the above valves to USCG and ABS for review and approval.

MEGI Silencer piping has been re-installed as per the approved repair proposal. The following items must be adressed to the satisfaction of the MI and ABS: - Update the drawing to reflect the as built piping installation - Provide the root cause of the failure of the fractured/cracked piping for the MEGI Silencer. - Conduct a vibration analysis of the new installed piping within 30 days of service.

Valves should be in accordance with the recognized standards taking into account the design pressure. Received notification that #2 LNG type "C" tank, liquid fill valve (FG505) bonnet is crack and leaking. Vessel is prohibited from LNG operations until a repair plan is received, reviewed, approved by OMCI Sector Jacksonville.

Vessel's crew identified a fracture on the aft LNG filling piping IWO FG706 and the N2 purge connection. Prohibit the use of the Aft LNG filling line until a repair proposal is submitted for approval, repairs are properly made and tested to the satisfaction of the attending Marine Inspector.

Complete repairs of the starboard LNG tank per the approved repair procedures; to include the flex spool piece installation, re-installation of the safety valves, and N7 valve and piping installation. Demonstrate proper operation of the LNG system to the attending CG Marine Inspector once all repairs are complete.

Repair/replace water spray pump coupling for the LNG system and demonstrate proper operation.

Properly install/mount the following Hazardous area devices IWO Turret Activation area/LNG Bunkering station: CCTV junction box (not mounted), and grounding straps missing/broken on junction box and light fixture. Also, re-attach ground straps at the LNG Bunkering station are required.

Replace/install the N2 generator bottles that were sent off for hydro.

Repair pipe patch on sewage vacuum line on stbd side engine room overhead IWO compressor controller.

Domestic Water Service (SS010) for D & E Deck hot water line has a pin hole leak in the cable trunk located on the D Deck.

Repack the fire block in the cable penetration from CCTV installation that penetrates the horizontal boundary from the cable trunk into the passageway on C Deck.

IMO Resolution MSC.285(86) requires components in remotesystems to be designed with redundancy or to be independent of each other, redundancy or independence shall also be provide in the control system. Received notification that the vessel Control and Monitoring System (CAMS) is not functioning as designed and the system is currently being operated in Manual mode.

Vessel's STBD LNG tank has developed another (Third) crack/leak within the angular space. Vessel's Port and STBD LNG tanks must be emptied and gas-freed as outlined in the proposal received (Second proposal) and accepted by Sector's Jacksonville and San Juan. CG-835V CLEARED AS OUTLINED IN THE ACCEPTED PROPOSAL..

A vibration analysis of the STBD LNG tank bellows (N13 and N14)must be conducted and results forward to USCG for review and approval.

The LNG Containment system drawing and arrangements must be updated to reflect the repair changes made to the following: N13 Piping and Bellows, and N14 Bellows. Updated plans must be submitted to class and USCG for review and approval.

The basis for type "C" independent tanks is based on pressure vessel criteria modified to include fracture, mechanics and crack propagation criteria. Ships crew discovered and reported the presence of methane being introduced into the STBD type "C" LNG tank annular space, (This is second leak within the annular space still Jan 23) and the annular space vacuum is decreasing (Losing vacuum). Vessel is required to remove all LNG from the port and starboard LNG type "C" tanks, gas-freed/inert tanks (Procedure must provide and approved prior to commencement), and provide an updated repair proposal for this event. Additionally, the vessel is prohibited for LNG operations to include bunkering until repairs are made to the satisfaction of the USCG Marine Inspector and class (ABS).

The basis for type "C" independent tanks is based on pressure vessel criteria modified to include fracture mechanics and crack propagation criteria. The Starboard LNG type "C" tank and annular spaces has been gas freed/inerted, and currently in an Nitrogen state. The following action must be taken until the tank is "safe for entree" to determine the actual cause/s of CH4 into the annular space : 1) Adhere to TOTE's vessel Starboard LNG type "C" tank and Annular Space Condition Sea Plan dated 28 Feb 2023. 2) Provide updates regarding the Starboard LNG type "C" tank and Annular space work/troubleshoot efforts prior to the vessels arrival Jacksonville, Fl 06 March 2023. 3) Maintain a positive pressure in the annular space of the starboard LNG tank below 1 psi per the Marine Chemist report, dated 28 Feb 2023. 3)Conduct rounds on the starboard LNG tank every 30 min, record reading of the manual gauge to ensure the pressure does not exceed 1 psi. and % of LEL/O2 @ the following sample points: lower, upper, and top. These reading must be forwarded to USCG (JAX) daily until the tank has been confirmed safe for entree by a certified marine chemist.

Permanently installed instrumentation shall be provided to detect when the primary barrier fails at any location or when the liquid cargo is in contact with secondary barrier at any location. The Starboard LNG type "C" tank annular space transducer and gauge are in fault and/or missing. Repair or replace as required prior to the tank being place back into service.

The basis for type "C" independent tanks is based on pressure vessel criteria modified to include fracture mechanics and crack propagation criteria. Ships crew discovered and reported the loss of vacuum in the angular space of the starboard LNG type "C" tank and methane is currently present. Actual cause/s cannot be found/determine without properly inerting/gas freeing the starboard tank to conduct further investigations. The following action must be taken: 1) The starboard LNG type "C" tank must be inerted/gas freed during the vessels next port stay Jacksonville, Fl (27th and 28th Feb 2023). 2) Maintain a positive pressure in the angular space of the starboard LNG tank below 1 psi per the Marine Chemist report, dated 20 Feb 2023. 3)Conduct rounds on the starboard LNG tank every 30 min, record reading of the manual gauge to ensure the pressure does not exceed 1 psi. These reading must be forward to USCG (JAX) daily until the tank has been inerted/gas freed.

A special maintenance manual should be prepared for electrical equipment that is installed in explosion hazardous spaces and areas. Marine inspector observed details of the electrical hazards plan was not fully implemented and procedures not included. 46 CFR 90.15, CG-ENG Policy Letter 01-12, Ref. IMO/MSC. 285(86), Reg. 8.3.3

Troubleshoot, isolate, and repair 440V A/C ground that indicated on main switchboard.

LNG storage tanks for liquid gas shuld not be filled more than 98% full at the reference temperature. Provide accurate loading/filling capacities for the LNG tanks via official document and/or clarification of the DSEC calculation of the filling limits for the LNG tanks (i.e. bunkering pressure/bars are incorrect as indicated on the chart, etc) additionally, vessels bunkering procedures need to be updated to reflect the above and current operations (i.e. LNG fUEL lOADING indicator, etc).

Properly Label all newly installed LNG valves/gauge lines(Port & Starboard tanks)and update the emergency/operations bunkering manual/s drawings as required.

FGS/CAMS migration FMEA/DVT and/or procedure has not been verified or approved. Once verification of the test procedures has been successfully witnessed/verified. The procedures must be submitted to USCG-MSC for review and approval.

LNG storage tanks for liqud gas should not be filled more than 98% full at the reference temp. Vessles port and starboard LNG tanks are in alarm condition (High level and over fill alarms active) and the radar system indictes 84%. Provide the following documentary prooof: Loading/filling capacties, whether or not the tanks can be loaded to the alarm set-points (85% and 90% respectively), and a report stating the tank measuring devices are reading correctly prior to the next bunkering ops.

LNG tanks PRV newly installed isolators (P&S tanks) interlocking mechanism per the approved drawing is not installed (04). Demonstrate proper installation once procured.

Port LNG tank feed pump hdyro activated valve (FG508) is leaking by and needs to be properly adjusted for the closure.

#2 FGS room ventilation outlet gas detector is in faulty and needs to be calibrated/replaced. Additionally, all gas detectors thru the vessel are due for calibration.

CAMS/FGS system displays/monitors must be updated to include the newly installed LNG valves, gauge lines, PRV isolators, etc.

Demonstrate the FGS nitrogen system O2 alarm set-point.

Newly installed LNG feed pumps (04) PSPT procedures need to be amended to reflect the under currect, ampage, etc.

Troubleshoot, isolate, and repair 120V A/C ground that indicated on the main switch board.

Remove dead-ended electrical cabling from the starboard LNG tank manifold.

FGS UPS (1&2) battery indicators does not indicate per design. During battery mode the indicator LEDlight goes out.

Precautions against shock, fire, and other hazards of electrical origin. Vessels electrical distribution (440V AC) ground detections systems indicates a dead ground. Make repairs to the satisfaction of attending MI/ABS surveyor. SOLAS 09 II-1/45

The condition of the ship and its equipment shall be maintained to conform w/ the provisions of present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or person on board. The vessel CO2 room detection (O2 sensor) does not function as per design. Make repairs to the satisfaction of attending MI/ABS surveyor. SOLAS 09 II-1/11

Gas Detection panel in ER indicates a system fault for the #2 Ventilation outlet of the FGS Room. Troubleshoot, affect repairs and demonstrate proper operation prior to operating on LNG to the attending [name withheld] 01-12 and MSC.285(86) 5.5

Electrical equipment in hazardous location must comply with the standard of the NEC and/or IEC. Vessels CCTV junction box located at the bunkering station found with a deadend electrical cabling. Make proper installation per the standard constructed to. CG-ENG Policy 01-12, Chapter 4

Oil residue (SLUDE) may be disposed of directly from the oil residue tank to reception facilities through the standard discharge connection. Vessel standard discharge connection (piping) has a small pinhole and needs to be repaired/replaced. Repairs must be completed and tested prior to the next discharge op. MARPOL I/12

The condition of the ship and its equipment shall be maintained to conform w/ the provisions of present regulations to ensure that the ship in all respects will remain fit to proceed to sea. MI found 02 penetration (01 into a bulkhead @ the portside mooring dk aft and 01 at the underdeck of the CO2 ROOM w/ missing insulation) that has not been approved and/or tested. Make proper proposal for approval and/or provide approved documentation prior to use of installations. SOLAS 09 II-1/11

Each gas storage tank should be provided with a tank valve capable of being remote operated and should be located as close to the tank outlet as possible. Vessel port LNG discharge feed pump valve, IFG 508, malfunctioned during test of an ESD activation.

Sector Jacksonville MI's were informed by the vessel Master that a main cylinder gasket on the main engine failed which affected the vessels fitness for service.

The approved Periodic Safety Test Procedures (PSTP) Rev. 1 to be updated to reflect the current equipment/systems on board the vessel. Numerous steps/details are not complete and/or not included in the procedures. EX: Loss of ventilation cap. for FSG rooms exhaust fan, etc. Once ABS has approved the PSPT, the procedures are to be submitted to the Marine Safety Center (MSC) for approval. Once MSC has approved the PSPT; shipboard testing of the PSPT to be witnessed by the cognizant Officer of Inspection (OCMI) or their designee prior to the annual inspection for 2021; 19Oct2021.

A block diagram showing change over procedures for remote steering gear control systems and power units shall be permanently displayed on the navigation bridge block diagram is not permanently posted on the bridge.

Aids fro escape shall be provided to ensure accessibility, clear marking and adequate design for emergency situations the Marine Inspector noted that an escape sign was being blocked and not clearly visible.

Means of control for the air supply to a space on the ship shall be provided to limit the fire growth potential. The Marine inspector noted a high oxygen content lever (24.7%) in the nitrogen generator due to a faulty supply fan.

Examples: Loss of vent cap. for the FGS rooms exhaust fans, proper id of spaces, DFDG's double wall piping gas detector inlet, and Preferential trips to name a few. Once the procedures are amended/updated, re-submit for approval from Class and USCG.

Troubleshoot and determine the cause of #3 DFDG not being able to operate on LNG (Gas)

#1 & 2 UPS for the FGS system are indicating fault conditons. Identify/correct faults and demonstrate proper operate.

Demonstrate proper operation of the newly installed LNG high pressure pumps ventilation system.

Install missing insulation (SFP) in GVT Room - Main Engine. SOLAS (2014) II-2/9.23. Outstanding Condition of Class.

Properly pack electrical cable for port aft winch in-way-of steering gear room. SOLAS (2014) II-1/40, Outstanding Condition of Class

Provide crew members pre-employmnet drug testing records. 46 CFR 16.210.

Vessel is prohibited from operating the SSDG's (3) on LNG until demonstrated satisfactory.

Vessel is prohibited from departing the port of Jacksonville until at least two Ship Service Diesel Generators (SSDG) are made operational and test satisfactory to the attending ABS surveyor and/or USCG marine inspector. Currently the vessel only has one generating set operating due to faulty exhaust valves on number one and three SSDG's.

Vessel Master failed to report inoperable S-Band radar on ANOA prior to entering the port of Jacksonville.

Vessel is prohibited from operating the ME-GI on LNG until the HP pump leak issues are rectified. Veesel is allowed to operate the ME-GI on LNG to verify installation/repairs and provide results to USCG for review and approval.

Conduct & demonstrate the Periodic Safety Test Procedures to the satisfaction of the attending Marine Inspector IAW ABS approved letter prior to USCG/MSC approval.

#1 LNG bunker line safety relief valve FG714 suspected of leaking by, cause methane of greater than 40% volume to be detected @ the vent mast riser detector. Vessel prohibited the use of #1 bunker line until rectified.

Vessel machinery propulsion/auxiliary alarms, (audible & visual)to include watch call, bilge s,etc. were found inop. Troubleshoot and determine the causes of the intermittently. Once rectified, demonstrate satisfactory ops.

#1 LNG bunker manifold relief valve FG713 found leaking during cool down ops in preparation for bunkering operations. Repair and/or replace the above relief valve and demonstrate proper operation to the satisfaction of USCG/ABS. Additionally, ispect/test #2 LNG bunker manifold relief valve FG733 and report condition to USCG/ABS.

Provide a repair proposal for #2LNG Bunkering liquid line that's leaking IWO FG731 for review and approval to USCG and ABS. Vessel is prohibited from bunkering ops. until repairs are made and tested satisfactory to the above parties.

Troubleshoot and determine the cause of hydraulic oil being emitted from the FGS cabinet room drain.

ME-GI GVT found defective (leaking) and needs to be repaired/repaired. Provide a repair proposal and timeline to USCG/ABS for review and approval. Once approve and repaired demo. satisfatory ops. of the GVT and ME-GI (gas mode) to the satisfaction of USCG/ABS.

#2 FGS high pressure pump room gas detector sampling system is in a fault condition and needs to repaired. Vessel is prohibited ME-GI gas ops unitl repaired and satisfactory ops is demonstrated.

Troubleshoot and determine whether or not the 120V and 440V A/C ground detection system is functioning as per design.

Replace/repair damaged sludge pump (pump has been removed).

Troubleshoot and determine the cause of hydraulic oil being emitted from the FGS cabinet room deck drains.

M/E J.C.F. JW and Low expansion tank coversand gaskets are damage and needs to be repaired.

Demonstrate proper operation of the Clean Water Separator system to include the audible and visual alarms once repaired.

Repair #1 and #2 FGS UPS cabinet door latching devices, once repaired demo that the latches work proper.

Troubleshoot and identify the component/s that's preventing #1 DFDG from shifting onto LNG (gas) mode. Once rectified, demo. satisfactory ops on gas and boil-off. Prior LNG opration.

Oily water separator system audible and visual alarms are inop. OWS may be used provided that the operator is positioned at the OWS local control while in use.

Troubleshoot and repair component/s that's causing an error code for the #1 cargo hold smoke extraction system as indicated on the panel.

Troubleshoot and repair #2Steering gear system Override feature. The Override feature is design to make temp. course change to avoid collision with other ships while operating in auto steering and currently not working.

Provide final copies of the operations, emergency and maintenance manuals to the local OCMI for review.

Troubleshoot, isolate, and repair 440V A/C ground on the main switchboard.

Properly pack electrical cable penetration, overhead portside of the purifier room.

Fire extinguisher in the FGS GVU spaces are not readily accessible and needs to be re-located within the spaces.

Investigate and determine the cause/s of the presence of gas/liquid of greater than 24% of volume being detected by the vent riser gas detector during bunkering operations.

Troubleshoot and determine the cause/s of the steering gear system rudder drifting to port with zero input while U/W non-follow up mode.

Repair/replace the faulty "A" silencer gas detector sensor for the ME-GI.

Troubleshoot and repair non-functioning heat detector starboard side aft of the LNG tank. Until repaired additional rounds must be made.

Properly align the ME-GI gas supply and return "purge lines". The purge lines to the engine are severely misaligned causing stress on the connections and gas leaks into the double wall piping.

Replace/repair faulty Scavenge Air Sensor PT8601 "b" air inlet receiver to the ECU.

Replace/repair faulty FGS/ESD air horn starboard side fwd, IWO the #2LNG tank.

Troubleshoot and repair issue/s preventing the USL fiber optic communication cable from functioning per design. Currently, the electrical USL system does function properly and being used as the primary.

Repair or replace leaking N2 generator three way valve.

Replace or repair non functioning sludge pump which only run in reverse direction.

Provide a detailed plan and timeline for the removal of the FGS Ventilation fans differential pressure swithes, etc.
