THE CONDITION OF THE SHIP AND ITS EQUIPMENT SHALL BE MAINTAINED TO CONFORM WITH THE PROVISIONS OF THE PRESENT REGULATIONS TO ENSURE THE SHIP IN ALL RESPECT WILL REMAIN FIT TO PROCEED TO SEA WITHOUT DANGER TO THE SHIP OR PERSONS ON BOARD. WHENEVER AN ACCIDENT OCCURS TO A SHIP, WHICH AFFECTS THE SAFETY OF THE SHIP, THE MASTER OR OWNER OF THE SHIP SHALL REPORT AT THE EARLIEST OPPORTUNITY TO THE ADMINISTRATION, THE NOMINATED SURVEYOR OR RECOGNIZED ORGANIZATION RESPONSIBLE FOR ISSUING THE RELEVANT CERTIFICATE, WHO SHALL CAUSE INVESTIGATIONS TO BE INITIATED. PSCOS RECIEVED REPORT THAT THE VESSEL COLLIDED WITH ANOTHER SHIP WHILE MOORING FOR A TRANSFER OPERATION. NO SURVEY HAS BEEN CONDUCTED BY CLASS SURVEYORS OR FLAG STATE SURVEYORS. THE VESSEL IS RESTRICTED FROM CONDUCTING CARGO OPERATIONS UNTIL THIS DEFICIENCY IS RECTIFIED TO THE SATISFACTION OF THE COAST GUARD. BASED ON THE PROVIDED ABS SURVEY REPORT, THIS DEFICIENCY HAS BEEN AMENDED. THE VESSEL IS PERMITTED TO DISCHARGE CARGO WITHIN THE SECTOR HOUSTON-GALVESTON CAPTAIN OF THE PORT ZONE ONLY. THE VESSEL IS NOT AUTHORIZED TO LOAD CARGO UNTIL REPAIRS ARE COMPLETED TO THE SATISFACTION OF THE VESSEL'S CLASSIFICATION SOCIETY AND THE COAST GUARD. 74 SOLAS (14) I/11 34-ABC- CARGO OPERATIONS RESTRICTED

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. 74SOLAS(04)I/11 The vessel experienced an equipment failure while transiting the navigable waters of the United States which adversely impacted its fitness and capability to safely maneuver the Houston Ship Channel. Submit for review a Classification Society report to [email withheld] attesting to the cause of the equipment failure, that authorized temporary or permanent repairs have been completed, and that the vessel's full and normal maneuverability has been restored.

A back-up arrangement should be provided facilitating means for safe navigation of the remaining part of the voyage in case of an ECDIS failure. Secondary ECDIS is inoperable. Vessel does not have an adequate back-up for safe navigation.

The machinery, boilers and pressure vessels associated piping systems and fittings shall be of a design...adequate for the service intended...to reduce to a minimum any danger to persons on board, due regard being paid to moving parts...Unapproved permanent arrangments have been made to the purifying system to allow for optional filtration.

The fire resistnace of doors shall be equivalent to that of the division in which they are fitted this being determined in accordance with fire test procedures code...doors in "B" class division shall be non-combustible. The accommodation doors leading to the lifeboat are missing steel, have holes in doors, and/or have temporary repairs.
