A vessel of not more than 7.9 meters (26 feet) in length must carry a minimum of one life buoy of not less than 510 millimeters (20 inches) in diameter Life ring was deteriorated and missing retro reflective tape.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. First aid kits and their contents were ruined due to water damage.

Each portable fire extinguisher on a vessel must be of an approved type. The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant OCMI. 10-B:C fire extinguisher had an expired servicing date.

All applicants for an original officer endorsement, except as provided in §§ 11.429, 11.456, and 11.467 of this part, must present to the Coast Guard currently valid certificate of completion of a CPR course from either The American National Red Cross, The American Heart Association, or a Coast Guard approved CPR course. Master's first aid and CPR course certificate was expired. 46 CFR 11.201(i)(2).

Rails or equivalent protection must be installed near the periphery of all decks of a vessel accessible to passengers or crew. Deck rails must include a top rail with the minimum height of 39.5 inches. Missing top safety chain on forward and starboard rail openings. 46 CFR 177.900 (a) and 46 CFR 177.900 (d)(1)

A vessel of more than 7.9 meters (26 feet) in length, but not more than 19.8 meters (65 feet), must carry a minimum of one life buoy of not less than 610 millimeters (24 inches) in diameter. Ring life buoy is unservicable and needs to be replaced.

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (2.5 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Vessel did not have fire buckets.

An adult life jacket must be provided for each person carried on board. Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. 01 life jacket had tear rendering it unserviceable, and 01 life jacket was not marked with the vessel's name.

A vessel on a lakes, bays, and sounds, or rivers route must carry three hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. Vessel's 03 handheld flares were expired.

Where flexible nonmetallic hose is permitted for use in piping systems by this section, it must meet SAE J-1942 (incorporated by reference; see 46 CFR 175.600) or be specifically approved by the Commandant (USCG Type A1, A2, B1, or B2). Nonmetallic piping for fuel system is dry rotted and needs to be replaced.

Portable watertight containers for distress flares and smoke signals shall be of a bright color, and containers shall be clearly marked in legible contrasting letters at least 12.7 millimeters (0.5 inches) high: “DISTRESS SIGNALS”. Container for distress flares was not marked in accordance with the above.

Gauges to indicate engine revolutions per minute (RPM), jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station. Engine gauges for engine RPM, jacket water discharge temperature (engine temperature), and lubricating oil pressure were either broken or missing.

A vessel must comply with the applicable requirements for any radio, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Master did not have Marine Radio Operator Permit. 47 CFR 80.159(e)

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel did not have a FCC Safety Radiotelephoney Certificate. 47 CFR 80.953

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. VHF radio does not have GPS feed into it. 47 CFR 80.905(a)(1)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. Starboard quarter has small gouge in the hull above the waterline.

An adult life jacket must be provided for each person carried on board a vessel. In addition, a number of child size life jackets equal to at least 10% of the number of persons permitted on board must be provided, or such greater number as necessary to provide a life jacket for each person being carried that is smaller than the lower size limit of the adult life jackets provided to meet this section, except that: (1) Child-size life jackets are not required if the vessel's Certificate of Inspection is endorsed for the carriage of adults only; or (2) When all “extended size” life preservers (those with a lower size limit for persons of 1,195 millimeters (47 inches) in height or weighing 20.4 kilograms (45 pounds)) are carried on board, a minimum of only 5% additional child size devices need be carried. Vessel required to replace two child and two adult sized PFDs to meet the carriage requirement of the COI.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. The four gasoline portable tanks were not safely secured to the vessel with a tie-down system.

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: (1) Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter) as amended here: (i) Certification or licensing by a state or local jurisdiction as a fire extinguisher servicing agency will be accepted by the Coast Guard as meeting the personnel certification requirements of NFPA 10 for annual maintenance and recharging of extinguishers. The annual maintenance was not conducted for the portable fire extinguishers by the owner or qualified third party. The 40: B/C requires replacement.

Lakes, bays, and sounds, and rivers routes. A vessel on a lakes, bays, and sounds, or rivers route must carry: (1) Three hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant; and (2) Three hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. Vessel is missing two USCG approved VDS.

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16). The new USCG ring life buoy requires retroflective tape.

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2), except that a VHF radiotelephone installation without DSC capability is permitted until one year after the Coast Guard notifies the Commission that shore-based sea area A1 coverage is established. Vessels in this category must not operate more than 20 nautical miles from land. Vessel's VHF was assigned an MMSI number but was not providing a lat and long. 47 CFR 80.905(a)(1)

A vessel must carry a first aid kit approved under approval series 160.041. First aid contents were expired. 46 CFR 184.710

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. The marking is not required on a life jacket carried to meet a temporary need for additional life jackets, if the life jacket has the name of another vessel or company marked on it. For an immersion suit, the name of the person to whom the immersion suit is assigned is an acceptable alternative to the name of the vessel. Vessel's life jackets not marked with vessel name. 46 CFR 185.604

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. The marking is not required on a life jacket carried to meet a temporary need for additional life jackets, if the life jacket has the name of another vessel or company marked on it. For an immersion suit, the name of the person to whom the immersion suit is assigned is an acceptable alternative to the name of the vessel. Vessel's ring life buoy not marked with vessel name. 46 CFR 185.604

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced Lifebuoy lifeline frayed and no longer serviceable. Replace with acceptable lifeline.

An adult life jacket must be provided for each person carried onboard a vessel. Passengers limited to 7 until non-serviceable life jackets are replaced and cleared by marine inspector.

A vessel on lakes, bays, and sounds, or rivers route must carry three hand orange smoke distress signals approved in accordance with 160.037 in sub chapter Q of this chapter, or other standard specified by the Commandant. Replace expired hand orange smoke distress signals.

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel’s name. Mark lifebuoy with vessel’s name.

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection. Ensure both fire extinguishers are properly secured with approved bracket.

A vessel must carry either a first-aid kit approved under approval series 160.041 of a kit with equivalent contents. Replace expired first aid kit with approved kit.

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operators station and the other at the access to the propulsion machinery space. No portable lights were located onboard.

A vessel not required to have a power driven fire pump by 181.300 must have at least three 2 1/2 gallon buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. No fire buckets were located onboard.

Each connection to a conductor or terminal part of a conductor must be made within an enclosure and have either: a pressure type connector on each conductor, a solder lug on each conductor, a splice made with a pressure type connector to a flexible lead or conductor; or, a splice that is soldered, brazed, or welded to a flexible lead or conductor. Spliced cable for trim tabs was located in the bilge and does not meet the requirements for cable connections. Propose new cable connection installation for repair.

At each vessel inspection all observed hazardous situations must be corrected. Vessel has cracks in aluminum overhead.

Fuel shutoff valves must be installed so as to close against the fuel flow. Install shutoff valve.

Vessels operating soley within 20 NM of land must be equipped with a VHF-DSC radio. Vessel is restricted to 1000 Feet from shore.

Portable fuel systems must meet the requirements of ABYC H.25. Ensure/prove Yamaha fuel tanks are approved or replace with ABYC H-25 approved.

Portable fuel tank must meet requirements of ABYC H-25. STBD Aft Tank is cracking, replace tank.

Remote fuel shutoff stations must be marked with 1" letters indicating purpose and direction.

Lifesaving equipment, including life jackets, must have the number and identification of the items stowed inside.

Portable fire extinguishers must be located so they are readily accessible. The installation must be to the satisfaction of the [name withheld] extinguisher with approved bracket.

Each small battery must be protected from falling objects. Provide terminal protection for batteries.

Batteries must be protected from roll and pitch of the vessel. Strap batteries to deck.

Connections to batteries must be made with permanent type connections. Spring type are prohibited.

Charts and nautical pubs must be adquate and up-to-date. Provide the following: charts, tide tables, current tables and rules of the road.

A durable placard must be posted next to all radio installations with emergency broadcast instructions.

The design and construction must meet ABYC H-25. Strap and secure fuel tanks.

Provide plan for repair of delaminated areas of hull.

Provide plan to repair blistered areas of FRP on port and starboard side.
