Suitable equipment and span gas should be carried onboard [to test/calibrate fixed gas detection]. Span gas bottle flow regulator was limited to .5 L/m; manufacturer instructions for fixed gas detection requires 1.5 L/m. Alternative test mehthod, via pillow, was found not suitable (not sufficient capacity) for testing from sample points on the vessel.

PSCO found broken seals on pressure relief valves for cargo tanks 4 port, 4 starboard, and 3 starboard. the remaining seals on other cargo tanks were not verified due to excessive paint on them. vessel is not authorized to load cargo tanks 3 and 4.

All pipelines or components which may be isolated in a liquid-full condition should be provided with relief valves. The handle to relief valve PR-V03 for condensate pipe at #3 cargo tank top was zip tied to prevent the handle from moving.
