Marine Employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crew members on inspected vessels. Vessel only had up to 2023 proof of enrollment. Provide current proof of enrollment for all employees in a safety sensitive position on the SALLY RIDE.46 CFR 16.230(a)

Each vessel must have a simple operating instructions with a block diagram, showing the change over procedures for remote steering gear control systems and steering gear power units, permanently displayed on the nav bridge and in the steering gear compartment. 33 CFR 164.35 (k)

The COTP or OCMI may require the vessel operator of any vessel that is required to have transfer procedures under § 155.720 to amend those procedures if the COTP or OCMI finds that the transfer procedures do not meet the requirements of this part. Transfer procedures found to contain inaccurate tank capacities/soundings in the description of the transfer system. 33 CFR 155.760(a)

Every oil tanker of 150 gross tonnage and above, and every other ship of 400 gross tonnage and above shall be subject to the surveys specified below: an annual survey within 3 months before or after each anniversary date of the Certificate, including a general inspection of the structure, equipment, systems, fittings, arrangements and material referred to in paragraph 1.1 of this regulation to ensure that they have been maintained in accordance with paragraphs 4.1 and 4.2 of this regulation and that they remain satisfactory for the service for which the ship is intended. Vessels IOPP Form A found to include tanks which have been removed from the vessel and with incorrect capacities listed for the remaining tanks. MARPOL Annex I/6.1.4

Waste receptacles shall be constructed of non-combustible materials with no openings in the sides or bottom. Plastic trash cans found in galley and scullery areas that require replacement. 74 SOLAS (20) II-2/4.4.2

All automatically or remotely controlled or monitored vital systems addressed by part 62 of this subchapter must be subjected to tests and inspections to evaluate the operation and reliability of controls, alarms, safety features, and interlocks. Test procedures must be submitted to the Coast Guard for approval. 46CFR61.40-1

Vessel had a blackout due to low fuel oil pressure. Identify the cause of the failure and enact a permanent repair. Cite: 46 CFR 188.20-1; 46 CFR 58.01-35.

The integrity of any deck in way of a stairway, shall be maintained by means of “A” or “B” class bulkheads and doors at one level. The integrity of a stairtower shall be maintained by “A” class doors at every level. The door shall be of the self-closing type. Holdback hooks will not be permitted. Holdback hooks found installed on the A60 fire screen door protecting the Port Auxiliary Machinery Room and the Starboard MSD Room. Remove unapproved holdback hooks. 46 CFR 190.07-10(d)(4)

Piping subject to internal head pressure from oil in the thank must be fitted with positive shutoff valves located at the tank. The quick closing fuel oil supply valve for the Port (#2) Fuel Oil Purifier failed to close upon remote actuation. Valve was overhauled and tested satisfactory. 46 CFR 56-50-60(d)

The general construction of the vessel shall be such as to minimize fire hazards. Fuel oil filters for Diesel Generators (#1-#4) missing metallic plugs on installed drain valves. 46 CFR 190.05-3

Each vessel which in the ordinary course of its trade makes voyages of more than 3 days duration between ports and which carries a crew of 12 or more must be provided with a hospital space. The hospital must have a toilet, washbasin, and bathtub or shower conveniently situated. Toilet in hospital space was disconnected due to work being performed. Captain stated crew was waiting on delivery of parts to reinstall the toilet. 46 CFR 190.20-35

Hydraulic power-operated steering gear shall be provided with a low-level audio/visual alarm for each hydraulic fluid reservoir with indicator relays on the bridge and engine control room. Steering pump B unit did not indicate any low-level hydraulic alarm when tested. Must rectify to the satisfaction of Class/Coast Guard and incorporate test procedures in the vessel PTSP.

All hatchways shall be fitted with hatch covers of steel or other equivalent material and such covers shall be water-tight. It was observed that a make-shift clamp was installed on cargo hatch cover to main engine space. Must show that cargo hatch cover meets water-tight standards to issuing ICLL authority.

Vessel currently has Periodic Safety Test Procedures (PSTP's) approved by DNV-GL and dated March 27th, 2017. Vessel is not classed with DNV-GL and numerous pen and ink changes have been made to the procedures. Submit new / updated PSTP's for approval. Approved for testing by next annual inspection.

Repair defective hose on the #3 SSDG oil supply crankcase breather housing. 46 CFR 190.05-3

Replace cracked oil supply crankcase breather housing on the #1 SSDG. 46 CFR 190.05-3

Install approved non-conductive electrical mat in front of ground detection panel in switchboard space. 46 CFR 111.30-11.

Remove all devices used to mislead water tight door indicators. Ensure indicator sensors work properly in all sea conditions. Calibrate sensors properly if failing in heavy weather. 46 CFR 170.270(e)

Correct IOPP Supplement to include Incinerator Sludge Tank as a means to collect oil residue. MARPOL 73/78 Annex I Regulation 12.

The machinery, boiler and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. The compressor or intake portion of the turbo and associated intake piping and manifolds for all four propulsion generators are not insulated posing a hazard to the persons on board as the surfaces were found to be at 238 degrees Fahrenheit during the time of the inspection. Provide heat insulation to all eight turbo intake portions piping and manifolds operating above 150 degrees Fahrenheit. SOLAS 74 (2014 Amend.) II/1/26.1

Each telegraph instrument must meet the protection requirements of 46 CFR 111.01-9 of this chapter. ( e) Each system must have an alarms which- (1) Automatically sounds and visually signals a loss of power to the system; (2) is on the navigating bridge; and (3) has a means to reduce the audible signal from 100 percent to not less than 50 percent IAW 46 CFR 113.35-5(d)(e).

The general construction of the vessel shall be such as to minimize fire hazards. Two locations were found in transducer room where structural fire protection for multi cable transits contained gaps/holes. Conducted proper repairs to fire boundaries. 46 CFR 190.05-3

Echo-depth sounder was not functioning during annual exam. Tech onboard, but could not fix issue.
