"Stairways and lift truncks which penetrate more than a single deck shall be surronded by at least "A-0" class divisions and be protected by self-closing doors at all levels." PSCO observed 4 self closing doors leading out of Stairway held open with wood blocks and 2 self closing doors on the bridge held open with rope.

"B" class divisions are those divisions formed by bulkheads, decks, ceilings or lining which comply with the following criteria: They are constructed as to be capable of preventing the passage of flame to the end of the first half hour of the standard fire test." PSCO observed a hole drilled in a "B" class fire boundary, between bathroom and VDR panle room on the bridge.
