The emergency generator shall be started automatically upon failure of the electrical supply from the main source of electrical power and shall be automatically connected to the emergency switchboard. When the vessel went dark-ship for an operational test the emergency generator did not start. 74 SOLAS CONS 2014 2-1 Reg 42.3.1.2

Structural fire protection including fire resisting divisions shall be kept in good order so as to ensure their required performance if a fire occurs. Multiple self-closing fire doors on the main stairwell were not self closing. 74 SOLAS CONS 2014 II-2 Reg 14.2.1.1.1

Cargo tanks shall be fitted with a visual and audible high-level alarm which complies with 15.19.1 to 15.19.5 and indicates when the liquid level in the cargo tank approaches the normal full condition. Vessel has CG-ENG mitigation letter to address IBC Code amendments dated 17FEB23 with an implementation due date by the vessel’s next dry docking following the date of the issuance of the letter. Vessel dry docked in DEC23 and was reissued a Certificate of Fitness by Class in APR24 and does not have the high-level alarm installed. The port/Starboard start air receivers had 200 PSI gauges installed and relief valve settings at 198. The gauges are lower than the required range. 46 CFR 54.15-5(f)

Each lens or diffuser of a lighting fixture must have a high strength guard or be made of high strength material. In the main machinery space one light aft of the start air receivers had a busted lens and one above STBD breaker panels was melted. 46 CFR 129.410 (a)

Each OSV must have, at the pilothouse, controls to start a fire pump and monitor the pressure in the fire main. During testing of the fire main, the pump pressure indicators were not reading correct values and showing only 02 PSI while the main was putting out adequate pressure. 46 CFR 130.430

Electrical equipment must be installed to minimize accidental personal contact with energized parts. Two breaker panels along the port bulkhead of the main machinery space were missing blanks, and there was a dead end wire on a distribution panel aft of the starboard air receiver. 46 CFR 129.200(c)

Nothing in this part shall be construed as limiting the inspector from making such tests or inspections as he deemed necessary to assure the safety and seaworthiness of the vessel. Vessels automation alarm light in the bow thruster space was inoperable, crew replaced the bulb and it was then made operable. 46 CFR 126.100

Design, construction, equipment, and operation of ships certified to carry NLS in bulk identified in Chapter 17 of the IBC Code, shall be in compliance with the IBC Code; vessel didn't have eyewash station or valves for sounding installed per letter 16714/2022-12-14T112434.

A self-propelled vessel of at least 65 feet shall have a properly installed AIS Class A device; AIS was operational but couldn't be updated manually.

Vessel must have an RAI at the steering control station; RAI (port rudder) at aft steering station in pilothouse reflected 05 degrees off while amidships.

(1) Nonmetallic flexible hose must be in accordance with SAE J1942. The starboard MDE fuel hose was cracked at the bends. Replace hose with fuel standard hose meeting standard above.

(a) Each vessel must be designed and constructed to minimize fire hazards, as far as reasonable and practicable. The fuel transfer meter and flange showed signs of leaking and presence of fuel. Repair and test.

(b) Each battery must be located as high above the bilge as practicable within the space the battery is located in and be secured to protect against shifting due to roll, pitch, and heave motions or vibration of the vessel, and free from exposure to splash or spray of water. The spare batteries in the E-gen space were not in containers and the posts were exposed. Secure and cover the posts.

The fire integrity of the openings and penetrations shall be maintained. The fwd engine room bulkhead ventilation penetration fire stop was wasted. Replace sealant at penetration.

Each lifebuoy line must be fitted with line resistant to deterioration. Lifebuoy forward of the house had dry rotted line, must be replaced.

Each lifebuoy must be marked in block capitol letters with the name of the OSV and name of the vessel’s port. Ring life buoy on the aft deck port side must be stenciled in legible block letters, to be re-stenciled.

Vessel must be provided with a recognized mobile satellite service ship station capable of transmitting and receiving distress and safety communications using direct-printing telegraphy. GMDSS printer not functioning, must be repaired.

All rescue boats must be properly constructed to be readily maneuverable. Rescue boats shall be constructed of materials accepted by the OCMI, as OCMI has final jurisdiction of construction. Fast rescue boat breaking through gel coat on upper port side. Must make repairs by an authorized party.

Gears, couplings, flywheels, and all machinery capable of injuring personnel shall be provided with adequate covers or guards. Aft rudder room p/s exhaust grates sustained substantial corroded from exhaust, must be replaced.

Indication of the rudder angle must be provided both at the main steering station in pilothouse and in the steering gear compartment. Rudder angle indicator in the steering gear compartment was inaccurate.

At least two (02) lifebuoys must be fitted with a self-activating smoke signal. Both bridge buoys had dryrotted bouyant line and the smoke signal was deattached.

Each liferaft must be arranged for free-float launching. The arrangement must ensure that the liferaft when released and inflated, are not dragged under by the sinking OSV. Four (04) of the six (06) liferafts were incorrectly configured to not be float-free and remain attached to the vessel, if it were to sink.46CFR133.310(b)(7)

Each vessel must have an amplifier-type announcing (PA) system that will supplement the general alarm. The PA system in the tank farm was inoperable.

Before any vessel leaves protected waters, the master shall ensure that the vessel's exposed cargo hatches and other opening sin the hull are closed; made properly watertight by use of gaskets; and properly secured for sea. MI noted that escape hatch from dry bulk tank farm to aft deck was not watertight; forward section of hatch had exposed sunlight coming through opening between knife edge & gasket. Owner shall make escape hatch watertight prior to departure.

Any ship of 400 GT and above shall be fitted with oil filtering equipment that will ensure that any oil mixture discharged into the sea after passing through the system has an oil content not exceeding 15 ppm. MI discovered that pre-wash tank of installed oily water separator onboard was covered with black algae and the oil content meter was unable to function as prescribed by the manufacturer. Owner shall provide evidence of proper operation of the system to include filtering.

MI noted extensive exhaust leak from port main diesel engine exhaust manifold. Owner shall repair leak to satisfaction of attending marine inspector.

In cargo ships of 2,000 GT and above, machinery spaces of category A above 500 cubic meters in volume shall be protected by an approved type of fixed water-based local application firefighting system. During testing of installed high pressure watermist system, the piping on the local pump station failed. Owner shall provide satisfactory operation of the installed fixed water-based local application firefighting system.

Each generator arranged for parallel operation must have reverse power or reverse-current trips. Owner shall provide proof of satisfactory reverse-power relays for two (02) installed generators.

Deadend wires found in the engine room starboard side in the vicinity of the fuel transfer pump. Wires need to be removed or put in a junction box. 129.200

Dead end wire found in the emergency generator room on the fuel tank just below generator. Wire needs to be removed or put in a junction box.

Nonmetallic hoses from lube oil sump to turbo have 1 clamp on each connection point. Each connection point needs to be double calmmped on each generator and forward thruster prime mover.

Vessel has unauthorized electrical equipment (Deck Lights) installed on deck in hazardous zones. ABS Electrical Hazardous Plan (Dwg E34 alt A0 02/08/17). Lights within the hazardous zones shall be Locked out/ Tagged out during flow-back operations and when independent tanks on the back deck are not certified gas free. Master must conduct JSA and ensure adequate lighting prior to night operations.

Emberkation lights at port side embarkation station found not functioning.
