Cargo tank relief-valve settings shall be a minimum of 0.02MPa gauge (2.9 PSI)IAW IBC Code/ approved CG ENG company mitigation plan prior to the carriage of methanol. 04 relief valves installed on the vessel are set to 14 kpa (2.0 psi). 705- Relief valve settings must be porperly set prior to carrying methanol.

At two lifebuoys, each with self activating smoke signals (approved under 160.157) fitted with lights, must be stowed where they can be quickly released from the navigation bridge. Port & starboard bridge buoy smoke signals fitted with lights were expired.

DP system suffered systematic error and vessel shall not conduct DP operations alongside or conduct cargo ops until DP system failure mode and effects analysis are completed to satisfaction of class and USCG.
