Unless otherwise expressly provided for in this subpart, the master, owner, operator, agent, or person in charge of vessels employing a Coast Guard-approved BWMS must meet the applicable ballast water discharge standard (BWDS), found in § 151.2030 of this subpart, at all times of discharge into the waters of the United States. USCG Sector New Orleans DMI received a report that the vessel has reported a malfunction in their ballast water treatment system due to a damaged UV Lamp and Sleeve. Vessel is restricted from discharging untreated ballast in waters of the United States until repairs are completed and a Class report attesting to satisfactory repair of the BWTS to Coast Guard Marine Inspectors.

MI observed water mist system activated in 3 zones - Stbd MDE, aft gen and fwd gen while testing remote activation test on the stbd MDE. Unless expressly provided otherwise, launching embarkation appliances complying with the requirements of section 6.1 of the code shall be provided for all survival craft. Where davit arms are recovered by power, safety devices shall be fitted which will automatically cut off power before the davit arms reach the stops in order to prevent over stressing the falls or davit, unless the motor is designed to prevent such over stressing. MI observed safety failed test of the limit switch.

The training manual, which may be comprise of several volumes, shall contain instructions and information, in easily understood terms illustrated wherever possible, on the life-saving appliances provided. The SOLAS Training Manual on board the vessel is missing vital ship specific lifesaving appliances and training pertaining to the vessel. Vessel to provide a corrective action plan to update the vessels SOLAS Training Manual to reflect the actual lifesaving equipment on-board.

Resolution: The Training Manual was immediately updated with current Davit details, corrected with proper instructions. Actions to prevent recurrence has been taken in place by the Company.

Forwartd and aft generators were found to have one nonmetallic expansion joint located above each expansion tank showing severesigns of deterioration. Inspectors marked joints with an X. Replace both expansion joints prior to departure.

All hatchways in position 1 and 2 shall be fitted with hatch covers of steel or other equivalent material. Except as provided in regulation 14(2)m such covers shall be weathertight and fitted with gaskets and clamping devices. 02 Engineroom dampers and 02 cargo room dampers did not have the proper gasket that will provide with a proper seal.

Requirement is 100 PSI on head and 150 on the line.
