All marine engineering details, such as boilers, pressure vessels, main and auxiliary machinery, piping, valves, and fittings, shall be designed, constructed, and installed in accordance with the provisions of this subchapter. Observed the Main Gas Turbine A had severely corroded enclosure walls leaving large gap on lower section rendering the enclosure ineffective for its respective CO2 fire fighting suppression system. Main Gas Turbien A shall not be used until the enclosure structure is restored to its original strength as designed.

Each applicant for approval of plans must submit three copies of each of the plans, specifications, and structural calculations concerning the construction, arrangement, required equipment, and safety features of the unit as described in 33 CFR 107.305. These include General Arrangement, Hull Structure, Stability, Fire Control, Marine Engineering, Electrical Engineering, Life Saving Equipment, Personnel Accommodations, Construction Portfolios, and the Operating Manual. The OCMI Office recognized on 4/14/22 that Portable Accommodation Modules (PAMs) were installed prior to CG Approval. Submit all approved plans for final review to the D8 OCMI Office and request inspection before occupying any of the three newly-installed PAMs.

An employee may request an exemption from the restrictions of employment in 33 CFR 141.15..The operating company authorized Sonoco to bring out persons without citizenship requirements or exemption letter. Prince is not an authorized facility. 33 CFR 141.20(a)

Lifeboats which are also rescue boats are required to have a data plate which states they have been prototyped to 46 CFR 156. Provide required data plate. 46 CFR 160.156-17

The emergency equipment provided... shall be maintained in good condition at all times. Monthly maintenance of releasing gear is not being conducted IAW manufacturers recommendations. Implement and provide feedback. 33 CFR 146.15

Each unit must carry at least one rescue boat approved under 46 CFR 160.156..and must have a rapid recovery winch. Prove by documentation or demonstrable evidence that winch meets rapid recovery standards. 46 CFR 108.560 and 108.570 (d)

The emergency equipment provided, shall be maintained in good condition at all times. Lifeboat A and B have wastage at lowering/recovery sheaves and lifeboat B has a broken/modified cable pulley mount. Provide NDE and evaluation report from a third party to verify the integrity of welds and modifications/repairs. 33 CFR 146.15

During testing of the emergency generator, MI's observed loose fuel rail brackets. Remove all fuel rail brackets, inspect fuel rail, and repair to the satisfaction of the attending USCG Marine Inspector.

During the inspection of the newly installed portable quarters, MI's observed the doors were listed as A60 Fire doors. However, no label was found and insufficient gasket material. Provide documentation attesting to fire class designation/rating and install to the satisfaction of the attending USCG marine inspector.

Lifeboats A&B are past 4 yr fall renewal date. Lifeboats taken out of service and standby vessel required until boats are placed back in service. Operational control issued.

Lifeboats A&B failed to start during inspection. Both boats already taken out of service due to fall renewal dates.

Lifeboats A-C have multiple repairs called out during last 3rd party servicing report with no resolutions >30days. Most notable were bad alternators on boats A&B and fuel leak on lifeboat A. It was noted that several smaller issues were noted, but no resolution taken. This resulted in an expanded SMS exam with BSEE to ensure the safety management system properly tracked maintenance and repairs.

Lifeboat C is to be taken out of service until air bottle is changed out/recharged. Current bottle has no pressure therefore boat has no internal air supply and lacks adeqaute fire protection. 33 CFR 146.15

Compressor control panels in hazardous location area is not sealed properly. Install control panel to Class I Div II requirements. PAJB5 Box has bare electrical wires. Install stuffing tubes to meet Class I Div II. [46 CFR 111.105]

Fire pumps A and B nitrogen bottles and guages are not functioning. Recharge bottles as needed (starting bottles) and replaces guages as needed. [46 CFR 108.415]

Post fire control plan [46 CFR 109.563(a)]

Galley door does not latch. Install proper latch and remove hold back. [46 CFR 108.137]

B Class doors in accommodation spaces are modified (latches missing, handle plates/locks missing or modified) and are required to be restored to original. [46 CFR 108.143(a)]

Accommodation and TLP electrical distribution panels are not labeled properly. [46 CFR 111.40-11 (a + b)

At least one door in the stairway is required to be self-closing/latching. Doors are modified and neither latch. Repair to original condition / replace. [46 CFR 108.143(a)]

Remove diesel fuel from lifeboat A bilge, clear drain plug of obstructing debris.

Grease sheaves IAW plan; provide maintenance plan for sheaves and falls. Sheaves squeeled excessveily during tests. BS stated that they never grease sheaves and solely rely on annaul maintenance by 3rd party. It had been 6 months + since last maintenance or inspection of davit, sheaves, falls, etc. [46 CFR 109.301(b)]

A-60 door in washroom TLQ does not latch; modified. Repair to original condition. [46 CFR 108.137]

Properly execute lock-out / tag-out procedures w/ distrubtion panels. [46 CFR 111.01-1(a)]

Multiple electrical panels on the production deck inside hazardous area do not meet classification requirments. Required to provide documentaion that panle are classified or meet classified requirements.46CFR111.105-3

Both fire pumps were leaking cooling water. Repair to original condition. 33CFR145.15

Lifeboats b and c had water in hull of boat from leaking windows. Return boat to original condition. 33CFR146.15

Release Hook on lifeboat B is not operating properly. Required to have 3rd party service the release hook and provide satisfactory report upon completion of service.

Engine in Lifeboat B will not start. Required to repair the starting system and have 3rd party verify condition and provide report to USCG upon service/repairs.

Lifeboat A has one bottle in the air support system that is extremely low. Reviewed reports from 3rd party servicing conducted in JAN12, which reported the same issue. Required to verify condition of bottle, and air lines, find the problem/air leak and complete proper repair to the system and recharge the bottle.

02 Lifebuoys on the Cellar Deck are inserviceable (severly faded). Required to replace lifebuoys with serviceable ones.

A new pressure vessel was installed on Fire pump #2. No CG stamp is on the tank, required to provide U1A form to verify construction of p/v.

Supports on P/V for Fire pump #1 are severly wasted. Required to repair supports on p/v.

Provide copy of Nabors Drilling final report of installation of work over drilling package.

Prove proper operation of fire pump"A" (diesel) fire pump must be operational prior to introduction of hydrocarbons.

Complete installation of drilling package, make ready for Coast Guard inspection. Complete to the satisfaction of an attending Coast Guard inspector prior to commencing drilling operations. See attached list

Prove proper operation of gas turbine generators operating on natural gas to the satisfaction of an attending Coast Guard inspector or ABS Surveyer.

Ensure proper operation and adequate water pressure from all fire hose stations for diesel powered fire water pumps.

Trouble-shoot & repair fire and gas detection system. Test to the satisfaction of an attending of an attending Coast Guard inspector or ABS Surveyor. Hourly rounds of affected areas to be made until this requirementis cleared. Log the same

Ensure proper operation of PCP Controls prior to commencement of introduction of hydrocarbons. Provide documentation of satisfactory tests.

Submit revised fire control and safety/ haz class area plans to reflect theinstallation of the drilling package and components.
