Tankers shall be equipped with at least one portable instrument for measuring oxygen and one for measuring flammable vapour concentrations, together with a sufficient set of spares. Suitable means shall be provided for the calibration of such instruments. PSCO observed both portable gas instruments were not functioning as design.

74 SOLAS (2009) II-2/9.4.2.2 Doors required to be self-closing shall not be fitted with hold-back hooks. However, hold back arrangements fitted with remote release devices of the fail-safe type may be utilized. Galley door is required by the fire control plan to be self-closing and was fitted with hold-back arrangements that required manual release from inside the space.

74 SOLAS (2009) II-2/9.4.2.1 The fire resistance of doors shall be equivalent of that of the division in which they are fitted, this being determined in accordance with the fire test procedures code... Doors fitted in boundary bulkheads of machinery spaces of Category A shall be reasonably gas tight and self-closing. Three (3) A-Class doors in machinery space were unable to close properly.

The purpose of this regulation is to provide means of escape so that persons on board can safely and swiftly escape to the lifeboat and liferaft embarkation deck. For this purpose, the following functional requirements shall be met: escape routes shall be maintained in a safe condition. Two of the escape trunks from the machinery space (aft steering) had seized latches/dogs and were unable to be manipulated.

No electrical equipment shall be installed in any space where flammable mixtures are liable to collect, e.g. in compartments assigned principally to accumulator batteries, in paint lockers, acetylene stores, or similar spaces, unless the administration is satisfied that such equipment is appropriated certified for safe usage in the dusts, vapors, or gases likely to be encountered. Three (3) of the explosion-proof lights in the cargo pump room were broken and exposed to the surrounding atmosphere.

Main and auxiliary machinery essential for the propulsion and safety of the ship shall be provided with effective means for its operation and control. While transiting to the berth at Centerpoint, the vessel suffered a reduction in maneuverability. PSCO received notification that the controllable pitch propeller controls in the engine control room malfunctioned.
