Controlled tank venting systems shall consist of a primary and a secondary means of allowing full flow relief of vapour to prevent over-pressure or under pressure in the event of failure of one means. Alternatively, the secondary means may consist of pressure sensors fitted in each tank with a montioring system in the ship's cargo control room or position from which cargo operations are normally carried out. Such monitoring equipment shall also provide an alarm facility which is activated by detection of overpressure or under pressure conditions within a tank. MI observed number 10 port cargo tank pressure indicator is not sounding alarm for both high and low pressure sensor testing. 74SOLAS(20)VII/10.1 IBC Code 8.3.3

In tankers, electrical equipment, cables and wiring shall not be installed in hazardous locations unless it conforms with standards not inferior to those acceptable to the organization. PSCO observed 04 self-lighting lights attached to ring life buoys not rated for use in a hazardous zones. 74SOLAS(20)II-1/45.11

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to ship or persons onboard. PSCO observed the ventilation in bosun stores tripped immediately upon being energized. 74SOLAS(20)I/11.

Each emergency generating set arranged to be automatically started shall be equipped with starting devices approved by the administration. Vessel reported that their hydraulic means of starting the emergency generator is not operational. This was confirmed by the PSCO while conducting the exam. 74SOLAS(20)II-1/44.2

Ships shall be provided with independently driven fire pumps as follows: in cargo ships of 1,000 gross tonnage and upwards, at least two. The vessel's fire and GC pump no.2 is not operational. 74SOLAS(20)II-2/10.2.2.2.2

The system shall be designed to ensure that if the oxygen content exceeds 5% by volume, the inert gas shall be automatically vented to atmosphere. The vessel's O2 analyzers are erratic readings. The vessel cannot conduct cargo operations until this is corrected. 74SOLAS(20)II-2/5.5.1.2 FSS CODE 15.2.2.2.4

Vessel reported a momentarily loss of reverse propulsion  due to #1 and #6 reversing cylinders not operating correctly.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Ship's Rescue Boat became inoperable during a man Overboard event occured on 12APR2020 during their transitto Charleston, SC from Spain. Fuel pump suffered damage rendering it inoperable. 74SOLAS (2014) Ch.III/Reg. 20.2. / LSA Code 5.1.1.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Due to event mentioned above, ship is missing 01 lifejacket, 01 immersion suit and 01 Man overboard buouy. 74 SOLAS (2014) Ch. III/ Reg. 20.2. / LSA Code 5.1.1.
