In a ship in which oil fuel is used, the arrangements for the storage, distribution, and utilization of the oil fuel shall be such as to ensure the safety of the ship and persons on board. PSCO observed fuel oil leaks and pooled oil on the three ship service generators and main engine. Additionally, numerous oil soaked rages were found presenting a significant fire hazard. 74 SOLAS 14 II-2/4.2.2.2

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. The vessel's crew failed to report nonconformities of critical items and functions IAW Chapter 9 of their SMS as indicated by deficiency #2. The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. The design shall have regard to materials used in construction, the purpose for which the equipment is intended, the working conditions to which it will be subjected and the environmental conditions on board. Temporary repairs were found in the following spaces: Piping for the Fire/Ballast/Bilge pump has 2 temporary repairs; 1 repair has failed and is actively leaking. Piping for the Fire/General Service pump has 2 temporary repairs; 1 repair has failed and is actively leaking. Seawater cooling pump piping has a failed, leaking temporary patch and main engine cooling line has 1 temporary repair. Main engine heat exchanger seawater piping has two temporary repairs. The freshwater generator had 2 temporary repairs and one pinhole leak. Generator heat exchanger has two temporary repairs.74 SOLAS(14)II-1/26.1, 30abc

Every steam pipe and every fitting connected thereto through which steam may pass shall be so designed, constructed, and installed as to withstand the maximum working stresses to which it may be subjected. PSCO identified multiple steam leaks from piping associated with the boiler system. Steam leak on the boiler, purifier flat and the #2 F. O. Tank steam piping on lowest deck of engine room. In addition, PSCO identified a temporary repair to the steam condenser for the cascade tank and weeping coming from the outboard side of the condenser indicating a material failure of the unit. 74 SOLAS(14) II-1/33.1, 17ac

Resolution: Received, reviewed and accepted class report from American Bureau of Shipping stating the deficiency was rectified. Techcross technician verified that the ECS-RCM rectifier control module was replaced enabling BWMS to operate in emergency local control.
