A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: (1) A light to indicate discharge;(2) An audible alarm that sounds upon discharge; and(3) A means to reset devices used to automatically shut down ventilation systems and engines as required by paragraph (a)(3) of this section. The indicating light, audible alarm, and reset device were inoperable. Repair the systems and demonstrate their function. 46 CFR 181.420(b)

A visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. The visual and audible high water alarms were inoperable in all three compartments. The visual bilge alarm was inoperable in the forward compartment. Demonstrate the function of the alarms. 46 CFR 182.530 (a)(c)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of lifesaving equipment and systems, including the following: Inspection of each hydrostatic release unit to determine that it is in compliance with the servicing and usage requirements of 185.740 of this chapter. The hydrostatic release for the EPIRB was expired and the batteries inside the EPIRB were expired. Replace the hydrostatic release and the batteries. 46 CFR 176.808 (a)(5)

The float free link for the life float must be: secured to the painter at one end, and to the vessel on the other end. The link was not secured. Secure the link to the vessel. 46 CFR 180.137 (b)(3)

Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly. Testing was not documented in the vessel log. Demonstrate evidence of testing. 46 CFR 185.728

The Master shall conduct frequent abandon ship, and man overboard drills to ensure crew familiarity with emergency duties every month but at least once every three month period. Drills must be formally recorded in the vessel’s official logbook, including the date of the drill and a summary of the instructions given. Drills were not documented in the vessel log. Demonstrate evidence of drills. 46 CFR 185.520

The inspection and maintenance of survival craft shall be logged or otherwise documented for review by the Coast Guard upon request. Maintenance was not documented in the vessel log. Demonstrate evidence of maintenance. 46 CFR 185.702(d)

Marine employers must ensure personnel directly involved in a Serious Marine Incident (SMI) undergo alcohol testing (breath, blood, or saliva) within two (2) hours. On board saliva swabs were expired, the breathalyzer was uncalibrated, and the personnel were not trained to administer tests. 46 CFR 4.06-15

The float free link for the life float must be: Certified to meet § 160.073 in subchapter Q of this chapter, or other standard specified by the Commandant. The link was missing the identification tag. Replace the tag. 46 CFR 180.137

Each ring life buoy must be marked in clearly legible block capital letters with the vessel's name. The name was worn. Rewrite the name. 46 CFR 185.604

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. The life jacket stowage location was not marked. Add the marking. 46 CFR 180.78

Provision must be made for closing all supply duct openings for a space protected by a fixed gas extinguishing systmen. Under seat air dampeners for the machinery space fire extinguishing system are blocked by PFDs, preventing rapid emergency closure. Clear PFDs from the dampener access area and stencil the closure locations for immediate visibility.46 CFR 182.465 (h)

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Three dead end wires were located under the operating station. Properly remove or cover the wires. 46 CFR 183.200 (c)

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. The life float had multiple fractures. Repair the fractures. 46 CFR 180.175

An adult life jacket must be provided for each person carried on board a vessel. Three adult life jackets were inoperable. Replace the life jackets. 46 CFR 180.71

Marine employers must establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who: Occupy a position, or perform the duties and functions of a position, required by the vessel's Certificate of Inspection. The vessel was found employing 02 crewmembers onboard that are not part of the chemical testing program established by the marine employer. 46 CFR 16.230 (a)(1)

A pre-engineered fixed gas fire extinguishing system must: Be installed in accordance with the manufacture's instruction. There are two openings one port and one stbd in the forward engine room bulkhead. Make the two openings vapor tight so the extinguish medium will not escape the engine room to the satisfaction of the attending marine inspector.

The Rules concerning lights shall be complied with from sunset to sunrise. Stbd navigation light not working. Restricted to daytime operations until replaced to the satisfaction of the attending marine inspector.

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. Hydrostatic release unit for EPIRB is expired. Vessel is restricted to operate within 3nm until replaced to the satisfaction of the attending marine inspector.

A hatch exposed to the weather must be watertight. Port engine room hatch gasket was torn where it seats on the knife edge. Replace to the satisfaction of the attending marine inspector.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Multiple dead-end wires were found underneath the main operating station. Rectify to the satisfaction of the attending marine inspector.

The following spaces must be equipped with a fixed gas fire extinguishing system, in compliance with § 181.410, or other fixed fire extinguishing system specifically approved by the Commandant, except as otherwise allowed by paragraph (b) of this section: A space containing propulsion machinery. Vessel's pre-engineered fixed gas fire extinguishing system bottle was found to be leaking contents. Bottle must be replaced and certified operable by technician. Testing of system to include engine shutdown and over ride must be witnessed by attending marine inspectors. 46CFR181.400(a)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Inspectors identified fracture in center void space on a structural longitudinal member on the port side of the vessel beneath the waterline. Crew must clean and expose area to identify extent of damage and needed repairs. 46CFR176.840

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: (1) Inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks. While inspection forepeak void, minor de-lamination was noticed on the port side where anchor line is stored. Repair de-laminated area to satisfaction of attending inspector. 46CFR176.802

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Port side damper does not latch closed and is missing pieces of gasketing which would create a tight closure when shut. Replace gasketing and adjust latch assembly so damper can be securely closed. 46CFR182.465(h)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Vessel crew created 02 inspection/cleaning ports on both the port and stb fuel tanks. No special consideration letter was issued to vessel and pressure testing of each tank never took place. Crew must submit documentation regarding alteration, any testing done, and prepare for pressure testing in the presence of attending marine inspector. 46CFR176.600

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. The equipment required for lifefloats and buoyant apparatus must meet the following specifications: The painter must be stowed in such a way that it runs out freely when the life float or buoyant apparatus floats away from a sinking vessel. Lifefloat painter line is stowed in metal apparatus which has a fractured metal guard. This guard, since broken, now leaves an exposed end which no longer guides the line but will instead catch when unspooling. Repair/replace metal guard to prevent catching of painter line during emergency. 46CFR180.175(f)(4)

Equipment specifications for life floats and buoyant apparatus. The equipment required for lifefloats and buoyant apparatus must meet the following specifications: (2) Paddle. Each paddle must be of at least 1,220 millimeters (4 feet) in length, lashed to the life float to which it belongs and buoyant. Paddles were not lashed to lifefloats. 46CFR180.175(f)(2)

In addition to meeting § 180.130, each life float and buoyant apparatus must be stowed as required under this section, (f) Life floats and buoyant apparatus must not be stowed in tiers more than 1,220 millimeters (4 feet) high. When stowed in tiers, the separate units must be kept apart by spacers. Vessel had life floats stacked with no spacers. 46 CFR 180.137(f)

Inspection and testing. All portable fire extinguishers, must be inspected or tested at least once every 12 months. Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Monthly checks were not logged or carried out. 46 CFR 142.240(a)(1)(ii)

When, in accordance with § 178.210 of this chapter, a vessel must be provided with a stability letter, the stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. If posting is impracticable, the stability letter must be kept on board in a weathertight container readily available for use by the crew and display to passengers and others on request. Vessel did not have COI or stability letter posted. 46 CFR 176.306

When operating a vessel on a body of water where the discharge of untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3, the operator must secure each Type III device in a manner which prevents discharge of sewage. Acceptable methods of securing the device include -(1) Closing each valve leading to an overboard discharge and removing the handle;(2) Padlocking each valve leading to an overboard discharge in the closed position; or(3) Using a non-releasable wire-tie to hold each valve leading to an overboard discharge in the closed position. Vessel did not have a means of securing MSD. 33 CFR 159.7(c)

All cable and wire must be protected from the weather. Dead end cable were found on the stbd aft section of the engine space. Remove dead end wires. 46 CFR 183.340(b)(3)

The manufacturer's instructions for onboard maintenance of survival craft, rescue boats, and launching appliances, manufactured on or after March 11, 1996, must be readily available for a vessel of not more than 19.8 meters (65 feet) in length. The vessel has a two inch by three inch area of wastage on the top most lifefloat. Repair as per manufacturers instructions. 46CFR185.702

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. vessel lacked instructions for heavy weather at the operating station. 46CFR185.510

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vessel must conduct fire and man overboard drills. 46CFR176.840

The machinery space must be equipped with a fixed fire extinguishing system. Vessel self-reported accidental discharge of fireboy while conducting maintenance. Vessel issued code 17 until fireboy system is serviced and certified to be operational.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards. The protective outer jacket of the wire used for the manual discharge of the fire boy was wasted and peeling off. The wasted part of the wire is located in the forward void.

An adult life jacket must be provided for each person carried on board. The vessel COI allows for 52 passengers; the vessel had 51 life jackets.

The number of life jackets must be marked and clearly legible where they are stowed. Life jackets stowed overhead did not display the correct number of life jackets.

The vessel must carry one floating water light with its ring life buoy. The water light battery was inoperable.

Each life float must be fitted with a light. Batteries for the water lights were inoperable.

When stowed in tiers, life floats must be separated by spacers. Life floats were missing spacers.

Each EPIRB must be tested monthly, and logged. EPIRB testing did not appear in vessel log.

Abandon ship, man overboard, and fire drills shall be conducted monthly, and logged. Drills did not appear in vessel log.

A hatch exposed to the weather must be watertight. The rubber gasket on the forward hatch was not properly attached.

A vessel on a limited coastwise route must carry six red distress signals and six hand orange smoke distress signals. Vessel was missing two distress signals.

A visual and audible alarm must be provided at the operating station to indicate a high water level in each normally unmanned space. Visual high level alarm was inoperable.

A vessel not required to have a power driven fire pump must have at least three 2 1/2 gallon fire buckets. Vessel was missing one fire bucket.

Connection must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. Both batteries contain wing nuts on the battery posts.

The master and crew of a vessel will be familiar with the content of and have mounted at the oprating station, emergency instructions containing the actions to be taken in the of fire, heavy weather, or man overboard condiitions. Vessel owner couldn't provided emergency instructions.

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, and the vessel from electrical hazards. Vessel had dead end cables at operating station.

Placards containing instructions for the donning and use of life jackets aboard the vessel must be posted, in place that are regularly accessible and visible to the crew and passengers. Vessel did not have a life jacket placard posted.

Abandon ship and man overboard drills and training shall be logged or documented for review by the Coast Guard upon request. The drill entry shall include the following information: Date of the drill and training, and general description of the drill scenario and taining topics. Details for drills and logs provided were insufficient.

Transmitters of any ship station must be performed by a person holding a commercial radio operator license or permit of the class required below. Current captain of the vessel could not provide a radio operator permit.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Both port and starboard main engines are inoperable.

Two (02) of the three (03) portable fire extinguishers need to be serviced.

Both life floats were not affixed with a float-free link.

Demonstrate proper functioning of bilge pump suction system

Secure Railing COnnections to canopy framing with appropriate hardware

Pump-out fwd void ahead of collison bulkhead and prepare for internal inspectoin

Provide working high water bilge alarm indication

Loose screws at the base of control console.

Provide drain holes in PVC conduit every 5 feet.

PVC electrical conduit is not sealed in engine room bulkhead.

Conduct minor fiberglass repair in compartment fwd of e/r transom.

5 fittings on hydraulic lines are corroded and need to be replaced.

Add positive shut off valve for the old generator overboard discharge line.

Replace the faulty cooling water through hull valve for Port MDE as the current one does not fully close.

After treating fuel in tanks, the high pressure fuel pump needed replacement.

Replace 03 non-UL submersible bilge pumps with UL-version (laz & E/R).

Properly repair delamination in forward compartment.

Provide draft marks in accordance with 46 CFR 185.602.

Provide 2014 edition of NAV Rules.

Repair or renew the navigation light cover for portside (red) light.

Properly repair the fractures located in the upper deck by the operating station for the awning framing.

VHF radio must be upgraded to VHF-DSC class A radio.
