C-ENDURANCE

Each vessel fitted with controllable pitch propellers, must have an indicator readable from the centerline conning position showing the pitch and operational mode of such propeller. Pitch indicator at emergency control station was defective and moving erradically.

Each enclosed space within the vessel must be properly vented or ventilated. Portside Cargo Area Supply Fan is inoperable.

An alarm system shall be provided indicating any fault, including an alarm for failure of the normal power supply of the alarm system. Nav lights work, but the alarm panel in the engine control room showed an active alarm for Navigation Light Power Failure.

Bunker delivery notes shall be accompanied by a representative sample of the fuel oil delivered. The sample must be retained under the ship's control for at least 12 months. Fuel samples for September 6 were missing. MARPOL VI/18.8.1

Entries shall be made in the Oil Record Book and shall be fully recorded without delay. Bunkering entries were made in Part II instead of Part I, certain entries were missing, and other entries were recorded inaccurately omitting exact fuel quantities.

Main and emergency switchboards shall be so arranged as to give easy access as may be needed to apparatus and equipment, without danger to personnel. The sides and the rear and, where necessary, the front of switchboards shall be suitably guarded. Exposed live parts having voltages to earth exceeding a voltage to be specified by the Administration shall not be installed on the front of such switchboards. Where necessary, nonconducting mats or gratings shall be provided at the front and rear of the switchboard. Space provided in rear of main switchboard is missing the nonconductive matting. All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. Vessel has three locations of dead-end wiring. 01 in vicinity of the f/o transfer appliances,01 in the access provided under the local steering controls of the after steering compartment and 01 in way of uninstalled sanitary pumps along the aft bulkhead of the machinery space. Simple operating instructions with a block diagram showing the change-over procedures for remote steering gear control systems and steering gear power units shall be permanently displayed on the navigation bridge and in the steering compartment. Instructions currently posted in the steering compartment do not accurately reflect change over procedure. SOLAS(14)V reg 26

The venting system shall be provided with devices to prevent the passage of flame into the cargo tanks. The design, testing and locating of these devices shall comply with the requirements established by the Administration based on the guidelines developed by the Organization. 03 F/O tank vents on deck had insufficient hardware to secure the flame screens. Periodic Safety Test must demonstrate the proper operation of primary and alternate controls; Vsl documented however failed to follow through on broken high coolant water temperature switch on the Port main engine approximately 120 ago; make corrections per approved PTSP

Vessel's oily water separator is required to meet the design and construction requirements of MEPC107(49). A manual recirculating facility is required to be installed immediately adjacent to oily water OVBD valve,installed in such a way so as to prevent by-pass of OWS. As currently configured, the manual recirculation valve is installed in such a way as to permit by-passing of OWS. MARPOL Reg 14-MEPC.107(49)
