Fill pipes and sounding pipes must be so arranged that overflow of liquid or vapor cannot escape to the inside of the vessel. Fuel tank in machinery space was fitted with hose attached to an open valve at the bottom of the tank, then zip tied in a vertical direction to determine fuel level. Valve was closed by crew and will be removed now the TLI is operational. 46 CFR 182.445(b)

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Visual indicator for the Machinery Bilge Pump was inoperable. Vessel will replace LED. 46 CFR 182.530(c)

At each initial and subsequent COI, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment to include operation of the fire main system. Fire station valve on 2nd deck leaking from valve stem, tighten or replace packing. 46 CFR 176.810(a)(3)

Vessel must carry one floating waterlight, unless limited to daylight ops. Waterlight did not have lithium batteries as called for by manufacturer's instructions. 46 CFR 180.70(d)

A hatch exposed to the weather must be watertight, except that the following hatches may be weathertight. A hatch on a vessel that operates only on protected waters. MI observed hatched leaking to engine room space. Rectify to the satisfaction of the Marine inspector. - 46 CFR 179.360(a)(3) 46 CFR 179.360(a)(3)

Inspection of each portable fire extinguisher, semi-portable fire extinguisher, and fixed gas fire extinguishing system to check for excessive corrosion or general condition. Engine room fire system pressure was found to be near the minimal acceptable pressure range. Replace bottle or install new engine room fixed fire extinguishing system by 16 May 2024. The Master shall check engine room fire system gauge in accordance with manufactures technical information bulletin TIB 08-2001-001 to verify pressure is within acceptable limits prior to each voyage with passengers. Each fire system check prior to a voyage shall be logged and made available to the Coast Guard upon request.

A self priming, power driven fire pump must be installed on each vessel of not more than 65 feet in length that carries more than 49 passengers. The vessels fire pump electrical system was inoperable and fire pump was disconnected. The electrical box that the fire pump is connected to needs to be repaired or replaced to the satisfaction of the attending marine inspector prior to movement and prior to the carriage of passengers.

An engineered fixed gas fire extinguishing system must be installed in accordance with the manufacturers instructions. The fire system content needle was outside the green. When questioned the vessel rep stated the system has a very small leak. Upon receipt of the tech report from IFS Fire & Safety INC. the report states the cylinder has lost pressure and is 4% low. The fire system needs to be repaired or replaced to the satisfaction of the attending marine inspector prior to movement and prior to the carriage of passengers.

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. The STBD generator belt guard is wasted away from rubbing on pulley and needs to be replaced. The port guard is also starting to waste away due to rubbing on pulley and needs to be repaired or replaced to the satisfaction of the attending marine inspector

A crew work space must be of adequate construction, and with suitable equipment to provide for the safe operation of the vessel and the protection and accommodation of the crew. The port-forward canopy leg on the bridge weather deck has separated from the wasted handrail on the port side upper deck. Repair or replace to the satisfaction of the attending marine inspector.

A self priming, power driven fire pump must be installed on each vessel of not more than 19.8 meters (65 feet) in length that carries more than 49 passengers. The power driven fire pump was inoperable, it worked for about 5 seconds then quit working and would not start back up.

Upper deck fire hose leaking through material. Replace fire hose to the satisfaction of the attending Marine Inspector.

A spanner wrench must be provided where a fire hose of 1.5 inch is required. Two spanner wrences are required for each fore hydrant.

Contents of the first aid kit were found expired. Replace items to meet requirments found in 46CFR184.710.

24NOV2017 - Prior to carrying passengers all items of the ship's outfit shall be ready for testing and inspection. The burnt cable for Stern thruster was replaced along with associated thruster unit(s) & equipment making both thrusters, Bow & Stern, operational. Conducted underway operational test of thrusters and noticed that the motor for Stern thruster was creating sparks as it was operated. Notified vessel captain of the situation and secured from further operational testing. Identify cause and repair / replace Stern thruster motor by a certified marine electrician to the satisfaction of attending USCG Marine Inspector.

14 NOV 17 - Prior to carrying passengers all items of the ships outfit shall be ready for testing and inspection. Vessel's bow and stern thrusters have become inoperable after removing burnt cable leading to the stern thruster. Repair/replace cable and associated thruster unit(s) and equipment. Provide documentation identifying the cause of burnt cable and equipment failure. Have a certified marine electrician install cable and equipment in accordance with manufacturer's instructions to the satisfaction of the attending USCG Marine Inspector.

Noticed running rust on starboard exhaust pipe in lazarette. Repair to satisfaction of attending CG marine inspector.

Identified burned wiring and cable insulation on power cable for stern thruster power. Identify cause and replace cable.

Cable through-runs were not packed, allowing free movement of vapors and gases between spaces. Pack through-runs to satisfaction of attending CG marine inspector.

Noticed rust on port rudder post. Identify cause and repair to satisfaction of attending CG marine inspector.

Wiring in aft steering gear room does not have fire stop leading through overhead bulkhead to bridge console. Fire hazards to be minimized. the general construction of the vessel must be such as to minimize fire hazards in so far as it is reasonable. PRIOR TO CARRYING PASSENGERS.
