A 6" puncture IWO port side, approximately 100' aft of rake and 7' above waterline. Vsl is to remain in empty condition and stay in port of Decatur Alabama to effect permanent repairs. Vsl is directed to contact local OCMI in Nashville, TN to arrange for inspection and repair verification. If conditions or plans change you are to contact the local OCMI.

Resolution: Proper repairs were made to hull w/ 18x 24" insert. Welder quals/procedures verified and tank pressed up to 1.5psi for over 30 minutes.
