A dry dock examination must be conducted while the vessel is hauled out of the water or placed in a drydock or slipway. During the examination all accessible parts of the vessel's underwater body and all through hull fittings, appendages, propellers, shafts, bearings, rudders, sea valves, and sea strainers shall be made available for examination. All vessels must have operable navigation lights in accordance with the Navigation Rules. All-around light not showing an unbroken light over an arc of the horizon of 360 degrees. Provide all-around light showing an unbroken light over an arc of the horizon of 360 degrees. 46 CFR 183.420

Gauges to indicate engine revolutions per minute (RPM) must be provided. Electronic engine gauges for port engine not operational. Provide operational gauges for port engine. 46 CFR 182.410(b)

Provision must be made for closing all supply/exhaust cowls for a space protected by a fixed gas extinguishing system. Fuel tank space supply cowls did not close and stay closed. Provide closure device for supply cowls. 46 CFR 182.460

A propulsion engine control system must be designed so that a loss of power to the control system does not result in an increase in shaft speed. Vessel is equipped with a fly-by-wire throttle system, however QFA and DVTP was never provided and test was not completed. Provide QFA and DVTP in accordance with CVC-Policy Letter 17-07 and conduct testing IAW approved QFA and DVTP. 46CFR184.620(c)

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 185.510 of this part and, when applicable, the duties listed in the station bill required by § 185.514 of this part. The vessel failed to properly record the required drills. Properly document drills. 46CFR185.420(a)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. The first-aid kit is missing the required Aspirin. Ensure first-aid kit has all required components. 46CFR184.710

A submersible electric bilge pump may be used as a power bilge pump only on a vessel of not more than 65 feet in length carrying not more than 49 passengers, other than a ferry, provided that the pump is listed by an independent laboratory as meeting the requirements in UL 1113. The vessel’s electric bilge pump is not UL rated. Replace installed bilge pumps with UL rated bilge pumps. 46CFR182.520(e)(1)

As appropriate for intended voyage, a vessel must carry adequate and up-to-date: Charts, U.S. Coast Pilot [name withheld] List, tide tables, and current tables. The vessels nautical charts are not corrected. Correct outdated charts or replace with current charts. 46CFR184.420

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel failed to complete credit drydock inspection within two years from last drydock inspection. Vessel completed credit drydock inspection. 46CFR176.600(c)(1)

Vessel name and hailing port sign missing from stern. Provide vessel name and hailing port on stern of vessel.

Hole located in deck in battery space. Repair hole to ensure fuel tank space is vapor tight and Pre-engineered fixed fire fighting system can cover cubic foot coverage area.

Discharge side of blower hose holed. Replace blower hose with new hose.

Ring buoy line deteriorated. Provide new ring buoy.

Items in first aid kit expired. Ensure all expired items discarded and replaced with valid items.

Supervisory and general crewmember drug awareness training not recorded/conducted. Conduct and record supervisor and general drug awareness training with staff.

Missing blower ignition interlock. Blower switch must be interlocked with the starting switch and the ignition switch so that the blowers are started before the engine started motor circuit or the engine ignition is energized. Provide blower ignition interlock.

Radio Safety Certificate not provided. Provide in accordance with 46 CFR 184.502.
