Issued 2 January 2021 Resolved
During a Marine Casualty in the morning hours of January 2, 2021 HSM 3216 Tank Barge obtained damage to the Port-side bow rake and the number 1 port void. Water egress was minimal but steady. Observed substantial damage to the internal structures in the bow rake, no pollution has been identified. It is unclear where the damage is but it is suspected that it is on the bilge knuckle between the bow rake and the #1 port void. Barge is authorized a one time discharge to the Marathon Facility and transit directly to dry-dock. Vessel crew are to conduct hourly rounds, have de-watering pumps on standby and notify local OCMI if conditions change. Barge must be inspected to the satisfaction of an attending marine inspector.

Action required: 15 - Rectify deficiencies by next port
Resolved 1 March 2021
Resolution: Attended vessel at Superior shipyard, Burlington, OH to verify repairs. Conducted air pressure tests and conducted internal structrual exams on the repaired areas. All satisfactory.
