Falls used in launching should be inspected periodically with special regard for areas passing through sheaves and renewed when necessary due to deterioration of the falls or at intervals of not more than five years, whichever is the earlier. During document review, found the five year servicing for all lifeboats was due in FEB 2023 was not done w/ no supporting documentation from flag/class and third party(Viking). Further, the release wire in LB#6 was called out to be repaired/replaced at the 5 year servicing as well. Provide USCG documentation supporting the extension, any mitigation plans, and plans for servicing. Further, provide proof of timely notifications to class/flag/coastal state IAW SMS & MODU Code.

During document review, found the rescue boat to be out of service. Provide supporting documentation from Flag/Class for this issue to include paperwork for a lifeboat certified as and in compliance with the code to be a Rescue boat and/or some other means of mitigation/rectification for this issue.

The condition of the unit and its equipment should be maintained to conform with the provisons of the MODU Code. The lube oil strainer for the #2 Diesel Generator in the Port Engine Room was fouind completely covered in waste oil. The waste oil actively ran along the side of the strainer creating a potential fire hazard. Crew secured the generator while CG was onboard to make repairs. Provided documentation attesting to proper repairs to the satisfaction of the adminsration. (Engine was energized during exam).

No change should be made to structure, equipment, fittings, arrangements and materials covered by the survey, without the sanction of the administration. A 1/4 inch jacket water line that runs to the #2 Diesel Generator header was found with a temporay repair to stop an excessive water leak. Despite the temporary repair, excessive water continues to leak from the line. Crew has secured the #2 DG in Port Engine Room to make repairs. Provide documentation atesting to proper repairs to the satisfaction of the adminstration. (Engine was energized during exam.)

Ships constructed on or after 1Jul2002 shall be fitted with navigation systems and equipment which will fulfill the requirements prescribed in paragraphs 2.4 to 2.9. The following systems had alarms at the time of exam: S-band & X-band radars, VDR, ECDIS, AIS, and DP system. Systems have been in alarm state for months. Operator requested servicing as per referenced email on 02 Mar 2018. 09 SOLAS V Reg. 19, 20. 50 a, c.

All lifesaving appliances must be in working order and ready for immediate use. Lifeboat air bottles were not set to the correct pressure to meet 10 minute positive air requirements in accordance with the MFGRs instructions. 09 MODU Code 10.18.1. 50 c.
