#2 LB was destroyed during the trip from New Zealand to the GOM. Each surface unit shall carry on each side of the vsl LBs that will accommodate the total number of people on board. Vsl is required to maintain a stby vsl until a new LB is installed and tested for boat and davit operation.

At least two separate means of escape should be provide, situated as far apart as possible. It was found that the heavy equipment room next to the weld shop and mechanics shop only has one means of escape due to the doors to the weld shop and the mechanics shop are locked with padlocks.
