PSCOs observed x02 fire doors in machinery space not latching properly and x02 fire doors had broken self-closing mechanisms.

The arrangements for the ready availability of water supply shall be in passenger ships of 1,000 gross tonnage and upwards such that at least one effective jet of water is immediately available from any hydrant in an interior location and so as to ensure the continuation of the output of water by the automatic starting of one required fire pump. When testing local application of water mist in engine purifier space, there was no ready availability of water.

Fire doors in main vertical zone bulkheads, galley boundaries and stairwell enclosures, except those for emergency escape trunks, shall be capable of remote release from the continuously manned central control station, either simultaneously or in groups, and shall be capable of release also individually from a position at both sides of the door. Release switches shall have an on off function to prevent automatic resetting of the system. PSCO observed fire door A.2.01 not release upon local activation.
