Each liferaft or group of liferafts shall be stowed with a float-free arrangement complying with the requirements of paragraph 4.1.6 of the Code so that each floats free and, if inflatable, inflates automatically when the ship sinks. The starboard liferaft is hooked up improperly to the hydrostatic release - on a permanent shackle and not through the weak link.

Maintenance, testing and inspections of life-saving appliances shall be carried out in a manner having due regard to ensuring reliability of such appliances. Liferaft # 11763211 had two required tests indicated on the service certificate that were past their due date.
